腹部Ⅱ类手术皮肤切口不同保护方法的效果比较

Effect comparison of skin incision protection methods in class Ⅱ abdominal surgery

目的比较腹部Ⅱ类手术皮肤切口不同保护方法的效果。方法选择行腹部Ⅱ类手术患者100例，按随机数字表法随机分为皮肤保护巾组和手术薄膜组，每组50例。皮肤保护巾组待腹部皮肤切开后使用皮肤保护巾，并妥善固定；手术薄膜组待手术区消毒液干后贴手术薄膜。两组均在皮肤消毒后、关闭腹部切口前皮肤采样，进行细菌培养，比较两组细菌培养结果及手术后切口愈合情况。结果两组患者皮肤消毒后细菌培养阳性例数均为0，皮肤保护巾组患者皮肤缝合前细菌培养阳性例数为11例，手术薄膜组为0，两组患者皮肤缝合前切口感染率比较差异有统计学意义（X^2=10．22，P〈0．05）；皮肤保护巾组患者手术切口愈合率为56．0％，手术薄膜组为100．0％，差异有统计学意义（x2=11．655，P〈0．01）。结论手术薄膜应用于腹部Ⅱ类手术切口有助于降低手术切口感染率，值得临床推广应用。

Objective To explore the clinical application of skin incision protection methods in Ⅱ class abdominal surgery. Methods Totals of 100 patients underwent abdominal operation of class Ⅱ randomly divided into skin protection towel group and the operation film group, each group of 50 cases. The skin protective towel group used skin protective napkin after abdominal skin incision, and properly fixed, while the operation film group posted operation film after disinfection liquid stemed. Collected bacterial colony counts of samples after disinfection, before closing abdominal incision, and postoperative incision healing situation was recorded. Results After disinfection, the positive cases of bacterial culture of two groups was zero. While before the closing abdominal incision, the positive cases of bacterial culture of skin protective towel group were Ⅱ cases and that of operation film group were 0 aases, and the difference was statistically significant （ X2 = 10.22, P 〈 0.05 ）. And the incision healing rate of skin protection towel group was significantly lower than that in operation film group （56.0% vs 100.0% ;X2 = 11. 655,P 〈 0.01 ）. Conclusions The using of operation film in class Ⅱ abdomen surgical incision can reduce the surgical incision infection rate, and worthy of being widely applied in clinic.
