小儿肺炎支原体肺炎341例临床分析

Clinical Analysis of 341 Cases of Mycoplasma Pneumoniae Pneumonia(MPP) in Children

目的探讨小儿肺炎支原体肺炎(MPP)的临床特征和治疗手段。方法将341例MPP患儿随机分为两组,分别给予静脉滴注阿奇霉素(196例)和红霉素(145例)治疗,定期观察两组患儿临床症状和体征改善情况,比较两组有效率和不良反应发生率。结果 341例患儿均有程度不同的呼吸道症状,其中134例有肺外临床表现。阿奇霉素组治愈率为98.98%,不良反应发生率为8.16%;红霉素组治愈率90.34%,不良反应发生率为35.86%,差异有统计学意义。结论 MPP常并发肺外脏器损害,阿奇霉素可作为小儿MPP的首选治疗药物。

Objective To analyze the clinical characters of mycoplasma pneumoniae pneumonia（MPP） in Children and explore available treatments on MPP in children.Methods The data of 341 children with MPP at the Second Affiliated Hospital of Nanjing Medical University was analyzed retrospectively.The hospitalized children were randomly divided into two groups,treated with azithromycin and erythromycin respectively.The clinical symptoms and the improving situation were observed,therapeutic effect and side reaction in two groups was compared.Results The respiratory symptoms existed in all cases and 134 cases had some extra-pulmonary complications.The cure rate of MPP was high in group with azithromycin than that in group with erythromycin（98.98% vs 90.34%,P〈0.01）,while the adverse effect rate of MPP was low in group of azithromycin than that in group of erythromycin（8.16% vs 35.86%,P〈 0.001）.Conclusion Besides the respiratory system,the extra-pulmonary organs could be detected during treatment with MPP.Azithromycin can be cured as the optimal drug for mycoplasmal pneumoniae.
