轮状病毒感染致小儿肠道外损害临床分析 被引量:13

Clinical Analysis of Extraintestinal Organ Damage in Children with Rotavirus Infection

摘要 目的分析轮状病毒性肠炎合并肠外感染的发生状况,了解轮状病毒性肠炎所致多器官损害的特点,探讨小儿轮状病毒肠炎合并肠外脏器损害的临床特征及其发生的可能机制,为临床医师认识该疾病提供理论依据,指导临床治疗。方法回顾性分析蚌医二附院2011年9月-2012年6月收住的144例轮状病毒肠炎住院患儿临床资料,采用胶体金法检测粪便A群轮状病毒抗原,阳性者144例入观察组,阴性者58例入对照组,观察两组呼吸、消化、循环、神经及泌尿、血液等系统损害的临床表现,实验室检验结果进行统计。结果观察组与对照组肠外脏器损害发生率比较,差异有统计学意义(P<0.05)。结论轮状病毒肠炎的RV病毒血症可能是RV多系统播散的途径,合并肠外损害具有普遍性,以呼吸系统损害最为常见,其次是心肌,肝脏,神经,泌尿及血液等系统。损害大多症状轻,恢复快,预后好,故在治疗轮状病毒肠炎时,需综合治疗,除了积极治疗原发病,要密切观察是否存在肠外脏器损害,防治严重并发症。 Objective To analyze prevalence of rotavirus enteritis with extraintestinal organ infection,explore the characteristics and mechanism of multi-organ damage due to rotavirus enteritis in children,and provide a theoretical basis for clinicians to understand the disease and guide clinical treatment.Methods Reviewed and analyzed the clinical data of 144 children with rotavirus enteritis in our hospital from September 2011-June 2012.The fecal group A rotavirus antigen was detected by Colloidal Gold Device assay.144 cases with positive result were selected in the observation group,and 58 cases with negative result were selected in the control group.The damage in respiratory,digestive,circulatory,nervous and urinary,blood system were observed and compared between the two groups.Results The difference in the incidence of extraintestinal organ damage between the two group was significant(P〈0.05).Conclusion The rotavirus enteritis combined extraintestinal organ damage is universal,of which the respiratory damage is the most common,followed by cardiac muscle,liver,and nervous system.Most of the damages are mild symptoms,recovered quickly and well.So it is important in the treatment of rotavirus enteritis to closely observe whether there is extraintestinal organ damage and prevent serious complications,in addition to the active treatment of the primary disease.
