电针结合耳穴放血治疗轻中度广泛性焦虑障碍的随机对照研究

Randomized control study on combination of ear acupoint bloodletting and electroacupuncture for treating mild to moderate generalized anxiety disorder

目的通过随机对照试验方法观察电针结合耳穴放血对轻中度广泛性焦虑障碍的疗效、不良反应及治疗依从性。方法将符合轻中度广泛性焦虑障碍诊断标准的66例患者随机分为治疗组和西药组各33例。治疗组采用电针百会、四神聪结合耳穴点刺放血,西药组口服盐酸帕罗西汀治疗。治疗前后采用汉密尔顿焦虑量表(HAMA)进行疗效评定、抗抑郁剂不良反应量表(SERS)进行不良反应评定、依从性问卷进行治疗依从性评估。结果治疗6周后,两组患者HAMA评分均较治疗前明显降低(P<0.01);组间比较,差异无统计学意义(P>0.05)。治疗组患者在第1、2、6周末的SERS和治疗依从性评分均低于西药组,差异有统计学意义(P<0.05)。结论电针结合耳穴放血治疗轻中度广泛性焦虑障碍的疗效与盐酸帕罗西汀相当,不良反应较少,且能有效提高患者治疗依从性。

Objective To observe the effect,adverse reactions and treatment compliance of the ear acupoint bloodletting combined with the electroacupuncture therapy in treating the patients with mild to moderate generalized anxiety disorder（GAD）.Methods 66 patients with mild to moderate GAD were selected and randomly divideded into two groups,33 cases in each group.The treatment group was treated by the electroacupuncture at Baihui and Sishengcong acupoints combined with ear acupoint bloodletting,while the Western medicine group received oral paroxetine.The therapeutic effects before and after treatment were e valuated by the HAMA scale.The adverse reactions were evaluated by the SERS scale.The self-designed therapy compliance questionnaire was applied to evaluate the treatment compliance.Results After 6-week treatment,the HAMA scores in both groups were significantly decreased than those before treatment（P0.01）,but statistical difference between the two groups was not observed（P0.05）.Besides,the SERS scores and the treatment compliance scores at 1st,2nd,6th weekend in the treatment group were lower than those in the control group with statistical differences（P0.05）.Conclusion Electroacupuncture combined with the ear acupoint bloodletting has the same effect as paroxetine for treating mild to moderate GAD,which has little adverse reactions and can effectively increase the treatment compliance.
