胆结石伴2型糖尿病87例临床特征分析

Analysis of clinical characteristics in 87 cases of gallstones complicating type 2 diabetes mellitus

目的探讨胆结石伴2型糖尿病患者的临床特征及预后,为胆结石的个体化治疗提供参考。方法采用病例对照方法分析2012年2—12月彬县县医院、彬县中医医院收治的87例胆结石伴2型糖尿病患者(糖尿病组)以及随机抽取的274例非糖尿病胆结石患者(对照组)的临床资料,对两组患者的临床特征及治疗预后进行统计学分析。结果两组患者在年龄、体质量指数、有无高脂血症、预后等方面比较,差异均有统计学意义(P<0.05),糖尿病组患者术后并发症发生率[72.4%(63/87)]高于对照组[15.0%(41/274)](P=0.001)。结论糖尿病是胆结石的高危因素之一,胆结石伴2型糖尿病的治疗更需个体化。

Objective To investigate the clinical characteristics and prognosis in the patients with gallstones complicating type 2 diabetes mellitus（T2DM） to provide reference for individualized treatment of gallstones.Methods By using the casecontrol method,the clinical data in 274 cases of non-diabetic gallstones and 87 cases of gallstones complicating T2DM treated in Binxian County Hospital and Binxian County Hospital of Traditional Chinese Medicine from February to December 2012 were col lected and performed the statistical analysis on the clinical characteristics,treatment and prognosis.Results The two groups had statistical differences in the aspects of the age,body mass index,hyperlipidemia and prognosis（P0.05）.The incidence rate of complications in the patients with diabetes mellitus complicating gallstones was higher than that in the patients with non-diabetes mellitus[72.4%（63/87） vs.15.0%（41/274）].Conclusion Diabetes is one of the high risk factors of gallstone and the treatment of gallstone complicating T2DM should especially need to be individualized.
