UF-500i尿沉渣分析仪细菌计数在指导临床送检尿培养中的作用

摘要 目的通过UF-500i全自动尿沉渣分析仪(简称UF-500i)对中段尿中的细菌进行定量分析,指导临床合理送检中段尿培养标本,合理使用抗生素。方法对700例新鲜中段尿标本做细菌培养菌落计数和UF-500i尿沉渣分析仪细菌计数进行比较。结果 122例UF-500i细菌计数>1000/μl时,细菌培养达到鉴定标准;569例细菌计数<100/μl时,细菌培养无菌生长或仅有几个菌落生长,培养未达到鉴定标准,判为阴性;有9例细菌计数介于100~1000/μl时,细菌培养3例阳性,6例阴性。结论当UF-500i检测出细菌数>1000/μl时可送检中段尿做细菌培养,以指导临床合理用药;UF-500i检测出细菌数<100/μl时无需送检中段尿培养;而当UF-500i检测出细菌数介于100~1000/μl时,可根据临床和UF-500i的另一些参数如WBC(白细胞)、NIT(硝酸盐)和尿沉渣涂片革兰染色镜检细菌来决定是否需要培养。 Objective To analyse the quantitative of urine bacteria by UF-500i automatic urine sediment analyzer (UF- 500i), guide the clinical reasonable inspection urine culture specimens, reasonable use of antibiotics. Methods 700 cases of bacteria on fresh midstream specimen of urine culture colony counts and UF-500i urinary sediment analyzer bacterial counts were compared. Results 122 cases of UF-500i bacteria count were 〉1 000/μ1, bacterial culture reached identification stan- dard; 569 cases of bacterial count were 〈100/μ1, no bacterial growth or only a few colonies cultured bacteria, bacteria colonies didn't reach the identification standard, sentenced to negative; 9 cases of bacterial counts were between 100-1 000/μl, bacteri- al culture were positive in 3 cases, negative in 6 cases. Conclusion When the UF-500i detects bacterial number 〉1 000/μl can be inspected mid-stream urine bacterial culture, and to guide clinical rational use of drugs; and UF-500i detection show that the bacteria number 〈100/μl without inspection urine culture. When the UF-500i detects bacterial numbers between 100-1 000/μl, according to clinical symptom and some other parameters such as WBC (white blood cells), NIT (nitrate) and urinary sediment smear gram staining microscopic examination to determine the need for cultivation of bacteria.
