小切口胆囊切除术在基层医院老年患者中的临床应用

Clinical application of small incision cholecystectomy in elderly patients in primary hospital

目的探讨小切口胆囊切除术在基层医院老年患者中应用的方法及效果,以探讨在基层医院老年患者中行小切口胆囊切除术的优越性。方法对2009年1月～2013年2月在本院开展的87例胆囊切除术进行回顾性分析,包括临床资料、术后恢复及随访结果。将患者分成两组,常规手术组42例采用传统手术切口,小切口手术组45例采用小切口手术。结果常规手术组在术后卧床、术后镇痛、抗生素使用、胃肠功能恢复的时间均长于小切口手术组,小切口手术组治疗效果较理想。结论在基层医院老年患者中施行小切口胆囊切除术治疗效果明显优于常规胆囊切除传统术式,经济效益优于腹腔镜胆囊切除术,在基层医院中具有实用价值。

Objective To investigate the method and effect for small incision cholecystectomy in elderly patients of primary hospital,in order to explore the superiority of the small incision cholecystecto-my for elderly patients in primary hospital. Methods 87 cases of cholecystectomy in our hospital from January 2009 to February 2013 were retro-spectively analyzed,including clinical data,postoperative recovery and follow-up results.Patients were divided into two groups,conventional surgery group in 42 cases with traditional surgery incision,small incision surgery group in 45 cases with small incision surgery. Results The bed after surgery,postoperative analgesia,antibiotic use,gastrointestinal function recovery time in conventional surgery group were longer than those of the small incision surgery group,small incision surgical treatment effect was ideal. Conclusion Small incision cholecyst-ectomy for elderly patients in primary hospital is superior to conventional cholecystectomy in treatment effect.It is also superior to tradition laparoscopic cholecystectomy in economic benefits.So,small incision cholecystectomy has practical value in primary hospital.
