IDA患者血清铁调素、铁幼素、膜铁转运蛋白和转铁蛋白受体2的表达特点及意义 被引量:7

Expression Feature and Significance of Hepcidin, Hemojuvelin, Ferroportin and TFR2 in Patients with Iron Deficiency Anemian

摘要 目的 研究缺铁性贫血(IDA)患者血清铁调素(hepcidin,Hep)、铁幼素(hemojuvelin,HJV)、膜铁转运蛋白(ferroportin,FPN)和转铁蛋白受体2 (trasferrin-receptor2,TFR2)表达特点,并探讨他们在IDA发病机制中的意义.方法 应用ELISA方法测定28例IDA患者和20例健康对照者的血清Hepcidin,HJV,FPN和TFR2浓度,并进行相关分析.同时还检测了血清铁蛋白(SF)和促红细胞生成素(EPO).结果 ①IDA组血清铁调素水平为2.04±1.27 ng/ml,明显低于对照组的3.07±1.68 ng/ml(t=2.203 6,P〈0.05).②IDA组的血清HJV水平为0.65±0.51 ng/ml,明显低于对照组的0.99±0.46 ng/ml(t=2.352 9,P〈0.05).③IDA组血清TFR2水平为8.15±3.09 ng/ml,明显高于对照组的6.62±1.42 ng/ml(t=2.061 7,P〈0.05).④IDA组的血清FPN水平为2.69±1.71 ng/ml略高于对照组的2.11±0.58 ng/ml,但差异无统计学意义(P>0.05).⑤SF与EPO呈负相关(r=-0.4958,t=2.911 0,P〈0.01),与HJV和Hepcidin均呈正相关(r=0.5015,t=2.9560,P〈0.01;r=0.515 7,t=3.069 1,P〈0.01).⑥Hepcidin与HJV呈正相关(r=0.398 3,t= 2.214 1,P〈0.05).结论 IDA患者高表达TFR2,低表达Hep和HJV.Hep和HJV在IDA的发病中有一定的意义,TFR2对IDA的诊断有重要的意义.联合Hep,HJV和TFR2对IDA与慢性病贫血等疾病的鉴别诊断有重要意义.FPN在IDA中的意义有待于进一步探讨. Objective To study the feature and significance of expression Hepeidin (Hep), Hemojuvelin(HJV),Ferroportin (FPN) and transferring-receptor 2(TFR2) in patients with Iron deficiency anemia(IDA). Methods To test the serum levels of Hep, HJV,FPN,TFR2 ,Erytrhopoeitein (EPO) and serum ferrin (SF) in 28 patients with IDA by with ELISA. Results ①The serum levels of Hep in patierts with IDA were 2.04-1-1.27 ng/ml,which were lower than 3. 074-1.68 ng/ml in control group (t=2. 203 6,P〈0. 05). ②The serum levels of HJV in patients with IDA were 0. 65±0. 51 ng/ml,which were lower than 0. 99 4-0. 46 ng/ml than in control group (t= 2. 352 9, P%0.05). ③The serum levels of TFR2 in patients with in IDA were 8.15±3.09 ng/ml,whieh were higher than 6.62±1.42 ng/ml in control group 0=2. 061 7,P〈0.05). ④The serum levels of FPN in patients with IDA were 2.69 4-1.71 ng/ml,which were no statistic differeney than 2. 114-0. 58 ng/ ml in control group (P〈0.05). ⑤There were negative correlation between Epo and SF (r = -- 0. 495 8, t = 2.911 0, P 0. 1) ,there were positive correlation between Epo and Hep and HJV (r=0. 501 5,t=2. 956 0,P〈0.01r=0. 515 7,t= 3. 069 1 ,P〈0.01). ⑥There were positive correlation both the serum levels of Hep and HJV (r=0. 3983 ,t= 2. 214 1 ,P〈 0.05). Conclusion In patients with IDA, the expression of TFR2 were higher,but the expression were lower of Hep and HJV. The expression of TFR2, HJV and Hep would be very significance for the anemia development in patients with IDA.
