Correlation of transcutaneous partial pressures of oxygen and carbon dioxide and arterial partial pressures of oxygen and carbon dioxide

摘要 目的探讨经皮氧(PTCO2)和二氧化碳分压(PTCCO2)监测值与动脉血氧(PaO2)和二氧化碳分压(PaCO2)的相关性。方法拟行开放胰十二指肠切除手术的患者16例,年龄38-68岁,ASAⅠ-Ⅱ级。在麻醉前(T1)、麻醉诱导后1h(T2)、2h(T3)、3h(T4)、4h(T5)同时记录并分析PTCO2、PTCCO2、呼气末二氧化碳分压(PETCO2)和PaO2、PaCO2值的相关性。结果PTCCO2、PETCO2和PaCO2以及PTCO2与PaO2在T1-T5各时间点均存在良好的相关性。PTCCO2与PaCO2总体线性回归关系:PTCCO2=17.70+0.42×PaCO2(r=0.644,P<0.01)。PETCO2与PaCO2总体线性回归关系:PETCO2=23.44+0.30×PaCO2(r=0.569,P<0.01)。PTCO2与PaO2线性回归关系:PTCO2=-10.07+0.99×PaO2(r=0.941,P<0.01)。结论在长时间手术患者,PTCCO2和PTCO2可以及时准确地反映动脉血PaO2和PaCO2。 Objective To investigate the correlation of transcutaneous partial pressures of oxygen(PTCO2) and carbon dioxide(PxcCO2 ) and the arterial partial pressures of oxygen(PaO2 ) and carbon dioxide (PaCO2). Methods Sixteen patients undergoing duodenopancreatectomy (aged 38-68 years old, ASA I Ⅱ ) were enrolled in our study. The PTC O2, PTC CO2, end-expired partial pressure of carbon dioxide(PETrCO2 ) and PaO2, PaCO2 (by blood gas analysis) were recorded at the same time points of before anesthesia(T1), at 1 h(T2), 2 h(T3), 3 h(T4), 4 h(T5) during anesthesia. The correlations among Prc CO2, PET CO2 and PaCO2 were analyzed. Results The PTc COz, PEr COz and PaCO2 were closely correlated at all time points of T1 to T5 (P(0. 01). The equations of linear regression were as follows. PrcCO2= 17.70+0. 42 ×PaCO2 (r=0. 644, P(0.01), PET CO2 = 23.44q+0. 30 × PaCO2 ( r=0. 569, P〈0. 01 ), and PTC O2 =-10. 07 + 0. 99 × PaOz ( r=0. 941, P〈0. 01 ). Conclusion PTc CO2 and PTc O2 can reflect PaCO2 and PaO2 timely and accurately in the patients undergoing surgery for a long time.
