口腔颌面部外伤性软组织缺损的即刻整形修复

Primary repair of traumatic soft tissue defects on oral and maxfllofacial region

目的探讨应用局部（邻位）皮瓣即刻修复口腔颌面部外伤性软组织缺损的方法与效果。方法自2003年11月至2012年5月，对37例面积2．0cm×2．5cm～4．0cm×6．0cm的口腔颌面部外伤性软组织缺损患者，根据创面缺损的大小、形状、深度、位置和邻近组织的特点，在特定解剖区域设计合适的改良菱形皮瓣、双叶皮瓣、鼻唇沟皮瓣、唇交叉组织瓣等多种方法即刻修复。结果本组所有皮瓣均成活，无明显的并发症发生，移植组织瓣的颜色和质地非常接近缺损部位的皮肤，功能和外形满意。结论口腔颌面部外伤性软组织缺损，在特定解剖区域设计合适的局部皮瓣即刻修复，能获得满意的面部外形和美观功能效果。

Objective To investigate the effect of regional skin flap in primary repair of oral and maxil- lofacial traumatic defects. Methods Based on the defect size, shape, location and the surrounding skin, totally 37 cases with facial tissue defects were repaired by modified rhomboid flaps, bilobed flaps, nasolabial flaps and Abbe-Estlander flaps in specific anatomical regions from November of 2003 to May of 2012. The size of these flaps ranged from 2.0 cm ×2.5 cm to 4.0 cm× 6.0 cm. Results All flaps survived well without any obvious complications. Repaired tissues were all matched well to the surrounding tissue. Resulting facial contour and appearance were good, and cosmetic results overall were satisfied. Conclusion Primary repair by constructing and transplanting regional skin flap for the regional conditions of facial tissue defect can obtain a good color, contour and texture match with the surrounding skin.
