超声引导腹横肌平面阻滞用于胃大部切除术后镇痛的随机对照研究 被引量:4

摘要 目的观察超声引导下腹横肌平面(transversus abdominis plane,TAP)阻滞用于胃大部切除术患者术后镇痛的有效性及安全性。方法40例择期行胃大部切除术患者按随机数字表法分为两组,每组20例,术后行超声引导TAP阻滞,双侧注射0.2%罗哌卡因各20ml(R组)或等量生理盐水(s组)。TAP阻滞完成后给予经静脉患者自控镇痛(patientcontrolled intravenous analgesia,PCIA)。记录术后2、6、12、24h疼痛视觉模拟评分(visualanalogue scales,VAS),术后24h内镇痛泵按压次数、舒芬太尼用量及相关并发症。结果术后2、6hR组的VAS显著低于S组(P〈0.05),24h内镇痛泵按压次数及舒芬太尼用量R组显著少于S组(P〈0.05)。两组均未见与TAP穿刺有关的不良反应。结论超声引导TAP阻滞用于胃大部切除术后镇痛安全有效,并可明显减少术后静脉镇痛药用量。
