摘要
目的探讨联合放化疗对乳腺癌患者的心脏损伤作用。方法乳腺癌术后患者60例,随机分成单一化疗组(单化组)和联合放化疗组(放化组),每组30例,单化组接受表阿霉素治疗,放化组接受放射治疗配合表阿霉素化疗,两组患者治疗前后均行动态心电图监测,并对监测结果进行统计学分析,比较两种治疗方法对心肌损伤的作用的大小。结果放化组24 h平均心率(88.7±7.3)次/分明显高于化疗组;出现心律失常7例、ST段改变4例、传导异常6例,共17例、总异常率为56.7%,明显多于单化组30.0%,两组比较差异有统计学意义(P<0.05)。结论联合放化疗比单一化疗增加乳腺癌患者的心律不齐、传导异常、ST段改变等心电异常例数,有增加心脏损伤作用,治疗时应定期行心电图检查,以策安全。
Objective To investigate the damage of chemotherapy combined with radiotherapy to the heart of the patients with breast cancer.Methods 60 postoperative patients with breast cancer were randomly divided into the single chemotherapy group and the group of combined radiation and chemotherapy,there were 30 patients in each group,the patients from the single chemotherapy group received epirubicin,the patients from the group of combined radiation and chemotherapy received radiotherapy and epirubicin,the dynamic ECG monitoring were performed for the patients from two groups before and after treatment and took the statistical analysis for monitoring results,then,compared the degree of myocardial damage from two treatment methods.Results The average heart rate(88.7 +7.3 times/min) of the patients from the group of combined radiation and chemotherapy at 24 hours was significantly higher than that of chemotherapy group;7 cases had arrhythmia,4 cases had ST segment changes,6 cases had conduction abnormalities with 17 cases in total,the total abnormal rate was 56.7%,which was significantly higher than that of the single chemotherapy group,there was statistical significance on comparing the difference between two groups(P0.05 ).Conclusion Combining radiation with chemotherapy can cause more abnormal ECG cases on arrhythmias,conduction abnormalities and ST segment changes in patients with breast cancer than that of single chemotherapy group,it has effect of increasing heart damage,therefore,ECG should be regularly taken on treatment to ensure safety.
