舒适护理对围术期剖宫产患者生理功能的影响

Effect of Perioperative Comfort Care on Physiological Function in Caesarean Section Patients

目的比较围术期剖宫产患者分别实施舒适护理和常规护理后对术后镇痛效果、睡眠质量、生理功能及术后并发症的影响。方法选择腰硬联合麻醉下行剖宫产的患者120例,根据不同的护理方法随机分为两组,舒适护理组(C组)和常规护理组(N组),每组60例。N组患者围术期常规予以基础护理及产科专科护理;C组患者围术期在N组基础上给予舒适护理治疗。观测术后泌乳时间、首次肛门排气时间和下床活动时间,评价48h内镇痛满意度评分,睡眠满意度评分;计算手术时间、术中出血量以及腹胀、便秘、尿潴留等不良反应的发生率。结果 C组48h内镇痛满意度评分,睡眠满意度评分高于N组,差异具有显著性意义(P<0.05),而C组的泌乳时间、首次肛门排气时间和下床活动时间、手术时间、术中出血量以及腹胀、便秘、尿潴留等不良反应的发生率明显低于N组,差异具有显著性意义(P<0.05)。结论围术期舒适护理能增加剖宫产患者术后的镇痛效果,改善患者的睡眠质量,减少手术相关的并发症,并能促进各项生理功能的早期恢复。

Objective To compare the effect on the analgesia, physiological function and postoperative complications in caesarean section patients undergoing comfort care and normal care during the perioperation. Methods 120 patients undergoing caesarean section in combined spinal-epidural anesthesia were randomly divided into two groups with sixty cases each： comfort care group（C） and normal care group （N）. The patients in two groups were received basic nursing and obstetric care. Besides, the patients in group C were using comfort care. Patient-controlled epidural analgesia was carried out after operation. The first time of lactation, anus exhaust and early ambulation were recorded. The scale of satisfaction degree in analgesia and sleeping, the operation time,the volume of blood loss and postoperative complications were calculated Results The scale of satisfaction degree in analgesia and sleeping in group C were significantly higher than those in group N（P〈0.05）, The first time of lactation, anus exhaust, early ambulation, the operation time, the volume of blood loss and postoperative complications showed much significant differences between two groups（P〈0.05）. Conclusion Perioperative comfort care has a better effect on patient-controlled analgesia and sleeping;it may be better to physiological function and can reduce postoperative complications in caesarean section patients.
