游离皮瓣移植修复大面积缺损的术后护理

Post-operative care of free flap for reconstructing large area defection

目的:观察游离皮瓣移植修复大面积缺损的术后护理效果。方法:选择接受游离皮瓣移植修复大面积缺损术后护理50例,手术后对患者实施系统的护理,包括术后的心理护理、一般护理、术后皮瓣的观察与护理及血管危象的判断及处理。结果:2例发生动脉危象,1例发生静脉危象,护士仔细观察、正确分析、报告医师后再次手术探查,重行血管吻合术、血管移植术,全部成活。结论:术后严密观察皮瓣血运、色泽、温度及肿胀程度,正确识别动、静脉危象并报告医生及时处理,以提高游离皮瓣移植的成功率。

Objective To observe the post-operative care effect of free flap for reconstructing large area defection. Methods Chose 50 cases accepting post-operative care, we gave them comprehensive postoperative care, including psychological care, general care, observation of the flap and judgment and treatment of vascular crisis. Results Two cases suffered arterial crisis, one case venous crisis, but the nurses observed carefully and correctly and reporting physician immediately, so all the flaps survived after surgical treatments. Conclusion Observation of the flap blood, color, temperature, and the degree of swelling closely, correctly identifying the artery and vein crisis then reporting to physician in time, can improved the success rate of free flaps.
