120例毛细支气管炎的病毒学检测和体液免疫变化 被引量:1

摘要 目的探讨毛细支气管炎的病毒病原学特征及体液免疫变化特征。方法将120例广东饶平及广东广州的毛细支气管炎患儿的呼吸道分泌物行呼吸道7种病毒检测,将此120例患儿(毛支组)及另外120例健康体检儿童(对照组)取外周血行体液免疫5项及IgE检测。结果在120例确诊毛支患儿的呼吸道分泌物呼吸道7种病毒检测中,57例为阳性,阳性率47.5%,其中以RSV为主,共43例,在病毒检阳性患儿中占75.4%。体液免疫检测中,毛支组IgE水平高于对照组,差异有统计学意义(P<0.05);毛支组水平IgG、C3、C4水平低于对照组,差异有统计学意义(P<0.05);两组间IgM、IgA水平差异无统计学意义(P>0.05)。结论毛细支气管炎患儿病毒检测阳性者以RSV为主,提示毛支病原谱稳定,预期今后相当一段时间内不会有明显变化;毛支患儿的体液免疫指标中,IgE水平高于健康儿童,提示其发生与人体超敏反应有关;毛支患儿IgE、IgG变化特点与儿童哮喘相似,提示两者间存在一定关系。 Objective To explore the viral etiology characteristics and changes of humoral immunity in patients with capillary bronchitis (CB). Methods 120 patients with capillary bronchitis were selected as the CB group to detect the 7 kinds of respiratory virus. 120 healthy children were selected as the control group. Both groups received the detection of 5 items of peripheral blood humoral immunity and IgE. Results The detection of 7 kinds of respiratory virus among 120 patients with capillary bronchitis showed 57 cases (47.5%) of positive, with the main kind of RSV (43 cases, 75.4%). The detection of humoral immunity showed that CB group had higher IgE level compared with control group (P 〈0.05); but the IgE, C3, C4 level in CB group were lower than those of control group (P 〈0.05); the differences of IgM and IgA were not statistically significant (P〉0.05). Conclusions Among the positive virus-detections, RSV is the most predominant one, indicating that pathogenic spectrum of capillary bronchitis keeps stable. In the humoral immunity detection, the level of IgE in patients with capillary bronchitis is higher than healthy children, suggesting that the mechanism of capillary bronchitis was related with hypersensitive response. The characteristics of IgE and IgG in children with capillary bronchitis appeares similar to asthma, implying that there is a certain relationship between capillary bronchitis and asthma.
