基于调查的针灸治疗肩周炎临床关注问题

Analysis of clinical concerns in treating frozen shoulder with acupuncture by clinical survey

目的:对针灸临床医生开展问卷调查以探讨针灸治疗肩周炎的临床关注问题。方法:以针灸临床医生为调查对象,围绕临床诊疗、个人经验展开针灸治疗肩周炎临床关注问题的调查和筛选。结果:所涉及的27个问题中,有26个问题关注度达50%以上,其中临床症状表现、诊断要点及取穴位居前3位;治疗方面:针刺+艾灸为临床最常用方法,肩周炎的针灸治疗应该早期介入,针灸治疗优势在于对疼痛的缓解;临床疗效评价方面:91.40%的被调查者选取综合疗效作为针灸治疗肩周炎临床疗效的评价指标,仅有不足3%的人能例举用于评价缓解肩部疼痛及功能活动受限的具体方法。结论:临床关注问题主要集中在疾病诊断、治疗与疗效评价3个方面;同时在疾病的中西医诊断、卫生经济学评价、疗效评价指标选择等方面尚存在分歧和不足,有待进一步规范和重视。

Objective：To investigate clinical focuses of treating frozen shoulder with acupuncture, surveys in acupuncturists were carried out. Methods：Questions in clinical treatments and personal experience were surveyed and selected among acupuncturists. Results：There were 26 questions having concern extent higher than 50% among those 27 questions. Clinical symptoms, diagnoses key points and acupoints selection were the top three hot questions. The combination of acupuncture and moxibustion was the most commonly used treatment for frozen shoulder by acupuncturist. They believed that the use of acupuncture in treating frozen shoulder should start early, and the effects of acupuncture were to relieve pain. And there were 91.40% acupuncturist chosing overall effects as their acupuncture effects assessment, there were no more than 3% acupuncturist who could figure out detailed methods to evaluate shoulder pain and movement restriction. Conclusion：Disease diagnoses, therapeutic methods and effects assessment are the top three clinical focuses in treating frozen shoulder with acupuncture. There are still discrepancies and draw backs in frozen shoulder diagnoses, health economics assessment, assessment methods selection, more attention and further standardization are needed in those areas.
