固本活血平喘方对慢阻肺急性加重期患者细胞外基质影响研究

Study on Influence of Guben Huoxue Pingchuan Formula on Extracelluar Matrix of Acute Exacerbation of Chronic Obstructive Pulmonary Disease

目的:从分子水平研究固本活血平喘方对慢性阻塞性肺疾病急性加重期(AECOPD)患者治疗的作用机制。方法:将入选的60例AECOPD患者随机分为2组,治疗组及对照组各30例,2组均给予抗感染、化痰、解痉平喘、氧疗等西医常规治疗措施,治疗组在此基础上加用固本活血平喘方,每日1剂,分2次服,每次200 mL。2周为1个疗程,连续治疗2个疗程后,观察2组患者的临床疗效、肺功能及肺纤维化三项含量的变化。结果:治疗组总有效率为93.33%,对照组总有效率为73.33%,两组有效率相比较,差异有统计学意义(P<0.05);治疗2疗程后,治疗组肺纤维化三项指标有所改变,与治疗前比较,差异有统计学意义(P<0.05);而对照组肺纤维三项无明显变化,与治疗前比较,差异无统计学意义(P>0.05)。结论:固本活血平喘方不仅有效缓解COPD症状,而且能够改善肺功能,延缓肺纤维化的进程。

Objective： The aim of this investigation was to evaluate the effect of Guben Huoxue Pingchuan Formula in the patients with acute exacerbation of chronic obstructive pulmonary disease （ AECOPD ）. Methods ： In the randomized trial, we had enrolled 60 consecutive patients with a recent history of AECOPD who were split into two groups equally （ Guben Huoxue Pingchuan Formula group and control group ）. All of them received a standard protocol consisting of the therapies of anti-inflammation, eliminating phlegm, anti-asthma and oxygen inhalation. Moreover, Guben Huoxue Pingchuan Formula group was allocated to additive 200 mL of Guben Huoxue Pingchuan Formula twice a day for two weeks until two courses of treatment. The outcomes including parameters of clinical efficacy, pulmonary function andfibrosis were observed. Results ： The clinical efficacy in the Guben Huoxue Pingehuan Formula group was 93.3% compared to 73.33% in the control group （ P〈0.05 ）. Postintervention for two courses of treatment, the significant decreases of three parameters of pulmonary fibrosis were observed in the Guben Huoxue Pingehuan Formula group （P〈0.05）when the control group remained unchanged （P〉0.05）. Conclusion： Adminstration of Guben Huoxue Pingehuan Formula improves pulmonary function and delays pulmonary fibrosis when used as an adjunct to standard therapy. Our results suggest that Guben Huoxue Pingchuan Formula could be a useful option in the treatment of AECOPD.
