宫内节育器和人工周期防治宫腔粘连的疗效比较与分析 被引量:5

摘要 目的:就宫内节育器和人工周期防治宫腔粘连的疗效进行比较与分析。方法:选取笔者所在医院2012年1-12月所收治的100例行子宫内膜多发性息肉电切术(TCRP)、子宫黏膜下肌瘤电切术(TCRM)、宫腔粘连电切术(TCRA)的患者作为研究对象,将其随机分为研究组和对照组,各50例。对照组在宫腔镜电切术完毕之后,在宫腔内放入"○"型节育环,而研究组在对照组的基础上,再加入人工周期。结果:50例对照组患者治疗后不孕及月经量过少4例(8%),单纯月经过少10例(20%),闭经3例(6%),周期性腹痛27例(54%);50例研究组患者治疗后不孕及月经量过少0例,单纯月经过少3例(6%),闭经0例,周期性腹痛7例(14%);50例对照组患者治疗后宫腔粘连分度Ⅰ级3例,Ⅱ级4例,Ⅲ级5例,Ⅳ级4例,Ⅴ级8例,粘连发生率48%;50例研究组患者治疗后宫腔粘连分度Ⅰ级2例,Ⅱ级3例,Ⅲ级1例,Ⅳ级0例,Ⅴ级0例,粘连发生率12%。结论:宫内节育器和人工周期联合防治宫腔粘连的效果远远优于单纯采用宫内节育器。 Objective: To compare and analyze IUD and artificial cycle efficacy in the prevention and treatment of intrauterine adhesions.Methods: 100 routinely admitted to our hospital From January to December 2012 between endometrial multiple polyps electrical excision procedure(TCRP), transurethral resection of submueous myoma(TCRM), intrauterine adhesions electrosurgical excision procedure(TCRA) patients as research subjects, were randomly divided into the research group, the control group, each group of 50 patients.Control group after hysteroscopic electrosurgery, putted " ~ " type IUD into intrauterine, the study group on the basis of the control group, then added artificial cycle.Results: 50 cases in the control group in patients with infertility and menstrual flow were too small 4 cases(8%), simple hypomenorrhea 10 cases(20%), amenorrhea three cases(6%), periodic abdominal pain in 27 patients(54%); 50 cases of the study group patients with too little 0 case, simple hypomenorrhea three cases(6%), amenorrhea 0 case, periodic abdominal pain in 7 cases (14%); 50 cases in the control group patients after treatment indexing grade I three cases, adhesions indexing IT grade 4 cases, adhesions indexing !1I grade five cases, adhesions indexing IV grade 4 cases, adhesions indexing the V level 8 cases the adhesion incidence of 48%; 50 cases of the study group patients after treatment of intrauterine adhesions indexing grade ] two cases, adhesions the indexing 1] level 3 cases, adhesions indexing I grade 1 cases, adhesions indexing level 1V 0 cases, adhesions indexing V grade 0 cases, the adhesion incidence of 12%.Conclusion: The effect of IUD intrauterine devices and artificial cycle in prevention of intrauterine adhesions is far better than simply using the IUD intrauterine devices.
