血清腺苷脱氨酶检测在妊娠期肝内胆汁淤积症诊断中的意义探讨

The significance discussion of serum adenosine deaminase detection in the diagnosis of intrahepatic cholestasis disease during pregnancy

目的：通过对妊娠肝内胆汁淤积症（IntrahepaticCholestasisofpregnancyICP）患者血清腺苷脱氨酶（AdenosinedeaminaseADA）的活性检测，探讨血清ADA检测在ICP诊断中的临床应用价值。方法：在美国贝克曼库尔特AU480全自动生化分析仪上检测实验组和对照组血清标本的ADA活性，同时检测肝功能相关的血清学指标（丙氨酸氨基转移酶（ALT）、L-y-谷氨酰基转移酶（GGT））、总胆汁酸（TBA）和总胆红素（TBIL）的水平，对其结果分析比较。结果：实验组所检测指标水平明显高于对照组，两组比较，差异有统计学意义和高度统计学意义（P〈O．05或P〈O．01）；不同孕期的ICP患者血清ADA、ALT、GGT、TBA和TBIL的水平呈递增性升高，趋势呈正相关，但血清ADA的相关性明显优于其他血清学指标，与其比较差异有统计学意义（P〈O．05o结论：血清ADA检测是ICP诊断的较敏感指标，可作为孕产妇筛查ICP的常规检测项目，具有较好的临床应用价值。

Objective ： Through the disease of Intrahepatic Cholestasis of pregnancy （Intrahepatic Cholestasis of pregnancy ICP） in patients with serum Adenosine deaminase （Adenosine deaminase ADA） activity test, to explore the serum ADA detection clinical application value in the diagnosis of ICP. Methods ： In the United States Beckman coulter AU480 full automatic biochemical analyzer test specimens in the serum of experimental group and the control group on the ADA activity, while detecting liver function related serological index （alanine aminotransferase （ALT）, L - gamma - pancreatic acyl transferase （GGT）, total bile acid （associates） and total bilirubin （TBIL） level, the results analysis and comparison.Results ： Experimental testing index level is significantly higher than the control group, two groups of comparison, the difference was statistically significant and highly significant （P 〈 0.05 or P 〈 0.01）; Different pregnancy of ICP patients serum ADA and ALT, GGT, associates and TBIL level increasing, trends were positively correlated, but the correlation of serum ADA is superior to other serological indexes, and compared the difference was statistically significant （P 〈 0.05）.Conclusions ： Serum ADA detection is more sensitive in ICP diagnosis index, can be used as maternal screening ICP regular testing prqiect, has good clinical application value.
