摘要
目的探讨糖尿病足溃疡感染患者的病原谱和耐药性状况。方法对65例糖尿病足溃疡感染患者分泌物进行病原菌培养和药敏试验。结果 65例患者分泌物标本中培养出病原菌122株,其中革兰阳性菌68株,革兰阴性菌51株,真菌3株;65例患者中39例混合感染。药敏试验显示多种抗生素交叉耐药,革兰阳性菌对万古霉素、复方新诺明、氯霉素、利福霉素较敏感;革兰阴性菌对亚胺培南为代表的碳氢酶烯及加β-内酰胺酶抑制剂的抗生素敏感。结论糖尿病足溃疡感染的病原谱分布广泛,对抗菌药物耐药率高,及时联合且有针对性选择敏感抗生素治疗能使患者受益。
Objective To study the spectrum of pathogens and their drug resistance of patients with diabetic foot ulcer. Methods Totally 65 patients with diabetic foot ulcer were enrolled and the exudate from the ulcer was collected for pathogen spectrum and drug sensitivity test. Results From 65 patients, 122 strains of pathogenic bacteria were isolated,of which,68 were Gram-positive bacteria,51 were Gram-negative bacteria,and 3 were fungi. 39 cases were polymicrobial infection.According to drug sensitivity test,multi-drug resistance was found.Gram-positive bacteria were sensitive to vancomycin, chemitrim, chloramphenicol,and rifamycin,while Gram-negative bacteria were sensitive to imipenem and antibacterial drugs with beta -lactamase inhibitor. Conclusion An extensive distribution of pathogenic bacteria in diabetic foot ulcer infection exists and The antibacterial drug resistant rate is high.Joint and the targeted choose sensitive antibiotic treatment in time can benefit patients.
