摘要 目的探讨肾动态显像测定慢性肾功能不全病肾小球滤过率的临床应用。方法慢性肾功能不全患者72例。该组采用99mTc-DTPA肾动态显像并检测GFR,同时测定BUN、Scr和Ccr。比较GFR与BUN、Scr和Ccr的相关性,及GFR值与Ccr值均数的差异性。结果该组患者GFR值与Ccr值相比较,GFR值明显低于Ccr值,差异有统计学意义(P﹤0.05);相关分析显示:GFR与Ccr呈正相关(r=0.712),GFR与Scr呈负相关(r=-0.624),GFR与BUN无明显相关(r=-0.118);慢性肾功能不全患者治疗前后GFR水平比较差异无统计学意义(P﹤0.05),治疗前后GFR变化不明显。结论肾动态显像方法简单灵敏,可用于慢性肾功能不全的肾功能判断,对治疗方案和预后判断有重要临床价值。 Objective To investigate the clinical applications of renal dynamic imaging to measure chronic renal insufficiency disease glomerular filtration rate. Methods 72 patients with chronic renal insufficiency were randomly divided into experiment group and control group, 36 cases in each group. In the experiment group 99mTc-DTPA renal dynamic imaging was used to detect the GFR, while in the control group BUN, Scr and Ccr were detected. The relativity of GFR and BUN,Scr and Ccr, the difference of GFR value and Ccr value of mean of the two groups were compared and analyzed. Results The GFR value of the experiment group was significantly lower than Ccr value, the difference was statistically significant (P 〈0.05). Relevant analysis showed there was a positive correlation (r = 0.712) between GFR and Scr, a negative correlation (r = -0.624) between GFR and Ccr and there was no significant correlation (r =-0.118) between GFR and BUN. These indicated that before and after treatment, the GFR level of the chronic renal insufficiency patients did not change significantly and the GFR level difference was not statistically significant (P 〈 0.05). Conclusion Renal dynamic imaging method is simple and sensitive, and can be used for the determination of renal function of chronic renal insufficiency, which has important clinical value for making treatment options and prognosis.
