上海市周桥社区“13533”家庭医生工作模式效果评价研究

Effect of Practicing Family Doctor Responsibility System in Zhouqiao Community Health Service Center of Shanghai

目的了解上海市周桥社区"13533"家庭医生工作模式的效果。方法选取春天花园居委会自愿与家庭医生签订《家庭医生服务协议》的社区常住居民421户共965人为本研究对象,采用陈华工作室现有"13533"家庭医生工作模式进行为期四年的健康干预。采用自制问卷,对研究对象进行家庭医生制服务知晓率和满意度的评估;随机抽取100名高血压患者进行遵医行为和高血压管理及控制情况评估。结果家庭医生制服务实施后,居民对家庭医生制服务的知晓率和满意度提高,高血压患者的遵医行为、高血压管理率及控制率均提高,与家庭医生制服务实施前比较,差异均有统计学意义(P<0.05)。结论家庭医生制服务提高了社区居民的满意度,改善了高血压患者的遵医行为,提高了高血压管理率、控制率,建立了良好的医患关系,家庭医生制服务符合社区卫生服务发展的需求。

Objective To investigate the feasibility and effect of practicing family doctor responsibility system in Zhouqiao community health service center of Shanghai. Methods Purposive sampling was taken in enrolling 965 residents from 421 households who were willing to sign the Agreement of Family Doctor Services in Zhouqiao community. The health intervention of the family doctor services continued for 4 years. A self - designed questionnaire survey was conducted among them about their awareness rate of the services and their satisfaction to the services. The condition of order - follow, the situation of management and control of hypertension were assessed in 100 hypertension patients who were randomly selected. Results After carrying out the family doctor system the rate of awareness of the residents and their satisfaction to the family doctor system increased ; and the behavior of hypertension patients in order - follow, the rate of management and control of hypertension were all significantly in- creased （ P 〈 0.05 ）, as compared with before the implementation. Conclusion The performance of family doctor responsibility system can increase the satisfaction of the residents, improve the order - follow behavior of the hypertension patients, raise the rates of management and control of hypertension, and establish a good doctor - patient relationship. So the family doctor responsibility system meets the demand for development of community health service.
