摘要
脂肪乳在肠外营养中有着十分重要的作用,能够提供能量和必需脂肪酸。目前临床上常用脂肪乳为50%中链三酰甘油(MCT)和50%长链三酰甘油(LCT)做物理混合的脂肪乳。结构脂肪乳是一种新型脂肪乳,是在同一个甘油分子骨架上同时具有MCT和LCT的重组三酰甘油。目前结构脂肪乳在动物实验及肠内营养研究中已得到肯定。肠外营养的研究中,结构脂肪乳对正氮平衡影响肯定,在代谢指标方面体现出更快的血浆清除率及加速氧化,但有的研究结果显示与普通的脂肪乳间差异无统计学意义。
Fat emulsion plays an important role in parenteral nutrition,it' s considered as a source of calories and essential fatty acids.Physically mixed 50% medium-chain triglyceride and 50% long-chain triglyceride is available in most Chinese hospitals now.Structured triglyceride(STG),a new type of fat emulsion,is defined as rearranged triglycerides that have both medium-chain fatty acid and long-chain fatty acid on the same glycerol backbone.STG is well accepted as a safe source of triglycerides in animal and human enteral nutrition studies.In parenteral nutrition trials,STG has a better effect on nitrogen balance,however,in terms of metabolic parameters,some studies have revealed faster clearance and greater oxidation for STG,whereas others revealed no marked difference comparing with common lipid emulsions.
