弹性髓内钉治疗儿童中重度桡骨颈骨折

Treatment of moderate and severe radial neck fracture with elastic stable intramedullary nail

目的观察弹性髓内钉(elastic stable intramedullary nail,ESIN)Metaizeau法及Metaizeau法联合撬拨复位在治疗儿童桡骨颈骨折中的效果。方法回顾分析19例桡骨颈骨折,平均年龄9.1岁;O'BrienⅡ型12例,Ⅲ型7例。单纯弹性髓内钉固定11例,弹性髓内钉结合撬拨复位8例。结果全部患儿均获经6～36个月的随访,优14例,良5例,无桡骨头骨骺早闭,缺血坏死,无关节周围异位钙化灶。结论弹性髓内钉Metaizeau法,弹性髓内钉Metaizeau法结合撬拨复位治疗儿童桡骨颈骨折,具有疗效可靠、微创、并发症少的特点。

Objective To investigate the effect of treating radial neck fractures in children with elastic stable intramedullary nail（ESIN）or Metaizeau combined with poking reduction.Methods A retrospective study was applied in 19 children with displaced radial neck fractures,O＇brien type Ⅱ 12 cases,type ⅡI 7 cases,all the fractures were reduced and fixed with ESIN,8 of them were treated combine with poking reduction.Results All cases were successfully followed up from 6 to 36 months.Fourteen cases were evaluated as excellent,5 cases were good.Epiphyseal closure,avascular necrosis and hetertotopic calcification were not found in any case.Conclusion Treating the radial neck fracture with ESIN or combine with poking reduction is a simple and minimally invasive method,the overall effect is good while the complications are limited.
