老年2型糖尿病合并尿路感染患者的临床特点及防治

Study on the clinical features,treatment and prognosis of type 2 diabetes mellitus combined with urinary tract infection in elderly patients

目的分析我院老年2型糖尿病合并尿路感染患者的临床特点及防治对策,为临床提供借鉴资料。方法选择我院2010年4月～2011年9月诊治的114例老年2型糖尿病患者作为研究对象,其中合并尿路感染的39例作为观察组,未合并尿路感染的75例作为对照组,分析两组患者的临床特点、治疗情况及预后。结果入院时观察组患者的空腹血糖、餐后2h血糖及糖化血红蛋白水平均明显高于对照组,且观察组病程明显长于对照组,观察组并发症发生率均明显高于对照组,组间差异均有统计学意义(P<0.05);对照组常规治疗、血糖控制达标率为86.7%,观察组在常规治疗基础上选用敏感抗生素治疗、血糖控制达标率为84.6%,尿路感染控制率达到97.4%,两组血糖达标率间差异无统计学意义(P>0.05)。结论老年2型糖尿病患者的血糖水平、病程以及并发症均为发生尿路感染的影响因素,医务工作者应了解我院老年2型糖尿病合并尿路感染患者的临床特点,以便实施更佳的治疗方案及预防措施。

Objective To analyze the clinical features,treatment and prognosis of type 2 diabetes mellitus combined with urinary tract infection in elderly patients and provide reference data for clinical.Methods From April 2010 to September 2011 in our hospital,114 cases of type 2 diabetes mellitus in elderly patients were selected as the object of study,39 cases with urinary tract infections as the observation group,75 cases without urinary tract infection as the control group,the clinical features,treatment and prognosis of two groups were analyzed.Results At admission,the level of fasting blood glucose,postprandial of 2 h blood sugar and HbAlC of the observation group were significantly higher than that of the control group,and the course of the observation group was significantly longer than the control group,the complication rates of the observation group were significantly higher than that in the control group,there were significant differences between two groups（P0.05）;the control group were treated with conventional treatment and the control compliance rate of glucose blood was 86.7%,the observation group added sensitive antibiotics based on conventional treatment and the control compliance rate of glucose blood was 84.6%,the control rate of urinary tract infection was 97.4%,the control compliance rate of glucose blood of two groups had no significant difference（P0.05）.Conclusion The blood sugar level,duration and complications are the influencing factors for type 2 diabetes mellitus combined with urinary tract infection in elderly patients,medical workers should be aware of the clinical features of type 2 diabetes mellitus combined with urinary tract infection in elderly patients in our hospital,in order to implement better treatment programs and prevention measures.
