摘要
目的:研究分析上海地区医疗机构临终关怀服务工作者和志愿者的认知、态度、行为能力,探讨国内医疗机构关怀服务的发展方向与构建模式。方法:选取上海市2家登记注册的临终关怀机构进行医疗机构问卷调查,同时采取随机抽样方法向社会大众发放问卷600份,分别从两个不同角度研究卫生资源和临终关怀供需及服务情况及社会大众对临终关怀基本知识、服务现状、服务意愿的看法。结果:上海地区人群对临终关怀基本知识认知度较好,对于临终关怀志愿者服务意愿和能力均具有一定基础;但医护人员对临终患者及其家属的心理需求关注度较低。结论:建立由社会工作者和心理辅导志愿者为骨干的"驻院"心理咨询师制度,以心理辅导和心理咨询服务形式提供无偿心理干预救济机制,并健全相关法律法规,完善临终关怀及心理干预服务模式。
Objec.tives: To investigate the cognition, attitude, and behavior ability of hospice-service workers and vohmleers in medical inslitutinns of Shanghai mnnieilmlity, and thrther to explore the development directions and construction patterns of hospital eare in China. Methods: Two registered medical institutions in Shanghai were assigned to complete questionnaires, and also a total of 600 questionnaires were delivered lo Ihe imhlie in random, Informatinn was collected with regard to the health resources, hospiee supply amt demand and its service, the puhlic atlihules towards fimdamenlal knowledge, service siluation and willingness, and was analyzed from two different aspects. Results: The cognition on fimdamenlal knowledge of hnspiee was generally hetter among Shanghai residents, and they also had certain fimndation fir the hospice-senqee willingness and ability of volmleers. However, the exlenl of psychological needs of terminal-stage patients and/heir families prmided by medical worker's was relatively lowered. Cnclusions: It highly advisable to establish the hospital-based psychological counseling institutions mainly involving social workers and vohmteers, who can provide neeessmy psychological intervention and remedy in the rehahilitation form or counseling at no charge. There is also a need to perfect related laws and regulations, and refine the service models of hospice and psychological intervention.
