彩超引导PTBD治疗肿瘤性梗阻黄疸的体会

Color dopplar ultrasound guide PTBD treatment for tumor obstruction jaundice

目的:探讨彩超引导经皮肝穿刺胆道置管引流(PTBD)治疗肿瘤所致梗阻性黄疸的临床价值及注意事项。方法:分析我院经彩超引导PTBD治疗168例肿瘤所致梗阻性黄疸的病例资料。结果:168例患者共行208次PTBD,一次穿刺成功率为92.3%(192/208),16例二次穿刺全部成功。PTBD后两周内88%患者黄疸显著下降,肝功明显改善,临床症状及全身情况明显好转;97%治疗有效。术中及术后无严重并发症发生。结论:彩超引导PTBD治疗梗阻性黄疸操作简便、经济、安全,具有定位精确、疗效显著、严重并发症少等特点,是目前姑息性治疗肿瘤所致梗阻性黄疸的安全、有效的方法。

Objective：To study the clinical value of color dopplar ultrasound guide percutaneous transhepatic biliary drainage（PTBD） treatment for obstructive jaundice caused by cancer.Methods：To analyze the data of 168 cases of obstructive jaundice caused by tumor treated by PTBD.Results：All 168 patients were treated with 208 times PTBD,success rate was 92.3%（192 /208）,16 cases with secondary puncture.In 88% of patients jaundice decreased significantly,liver meritorious service was improved obviously,clinical symptoms and general condition improved obviously.There was no intraoperative and postoperative serious complications.Conclusion：Color dopplar ultrasound guide PTBD treatment for obstructive jaundice is simple,economic,safe,with accurate location.
