宫腹腔镜治疗子宫纵膈144例临床分析

Clinical analysis of 72 cases of laparoscopy in treatment of uterine mediastinum

目的研究在宫腹腔镜下治疗子宫纵膈的临床效果。方法该院选择2010年9月—2012年9月间144例在宫腹腔镜下治疗子宫纵膈患者的恢复情况以及术后患者的妊娠情况。结果本文所选的患者均一次完成手术,未见有并发症出现,手术3个月后对其进行复查,超声显示宫腔形态正态。对这些患者进行9～42个月的随访,其中116例成功妊娠,受孕率达到80.6%(116/142),妊娠时间距离手术时间为5～12个月;妊娠的妇女有12例流产,流产率为10.3%(12/116);80例产妇足月分娩,其中32例为胎膜早破,20例为臀位。成功分娩的产妇均健康,未见胎儿畸形及低出生体重儿。结论在宫腔镜下对子宫纵膈进行切除是治疗子宫纵膈的良好方法,能够提高妊娠的成功率和胎儿的健康率。

Objective To study the clinical effect of treatment of uterine mediastinum in laparoscopy. Methods In our hospital from September 2010 -2012 year in September 72 cases of patients with uterine mediastinum in laparoscopy recovery and postoperative pregnancy. Results The patients were chosen in this paper to complete the operation, no complication, operation 3 months after the review, ultrasound showed a normal uterine cavity form. 9-42 months of follow-up of these patients, including 58 cases of successful pregnancy, pregnancy rate 80.6% （ 58/72 ）, the pregnancy time distance operation time was 5-12 months; pregnancy 6 cases of abortion, abortion rate was 10.3% （ 6/58 ）; 40 cases were full-term delivery, including 16 cases of premature rupture of fetal membrane, 10 cases of breech presentation. The successful delivery of maternal health were, no fetal malformations and low birth weight infants. Conclusion In hysteroscopic resection of uterine mediastinum is a good method in the treatment of uterine mediastinum, can improve the success rate of pregnancy and fetal health rate.
