经皮椎体成形术治疗新鲜骨质疏松性椎体骨折63例

摘要 目的探讨经皮椎体成形术(Percutaneous Vertebroplasty,PVP)治疗新鲜骨质疏松性椎体压缩骨折方法及临床疗效。方法回顾性分析PVP治疗63例新鲜骨质疏松性椎体压缩骨折,术前及术后2d进行疼痛视觉类比评分﹙VAS﹚、测量术后伤椎高度增加,随访伤椎高度丢失情况,记录骨水泥渗漏及随访期间相邻椎体骨折情况。结果术前及术后2dVAS分别为(6.8±1.3)分和(2.0±0.6)分,伤椎高度较术前增加(2.3±1.8)mm。术后随访6~24个月,平均13.3月,患椎高度丢失不明显,相邻椎体再骨折3例,无患椎再骨折,无感染、无骨水泥渗漏等并发症。结论经皮椎体成形术治疗新鲜骨质疏松性椎体压缩骨折有效、安全、减少长期卧床并发症,提高患者生活质量、值得推广。 Objective To assess the clinical effects and methods of percutaneous vetebroplasty for osteoporotic vertebral compression fractures. Methods 63 patients with OVCF undergoing PVP were reviewed, The considered factors included the VAS of preoperative and postoperative after 2 postoperative days , and the height of the injured vertebra of preoperative and postoperative, the PMMA leakage and adjacent vertebra fractures. Results The VAS improved from a mean preoperative score of (6.8±1.3) to a mean postoperative score (2.0±0.6) and the height of injured vertebra increased (2.3±1.8)mm after 2 postoperative days .All patients were followed up 13.3 months on average (from 6~24months).There were adjacent vertebra fractures in 3 vertebrae in 3 patients, and the postoperative loss of height was not obvious.There was no injured vertebra fracture or infection or PMMA leakage or other complications. Conclusion PVP provides significant pain relief in patients with fresh osteoporotic vertebral compression fracture, and it is effective and safe for patients with fresh osteoporotic vertebral compression fracture, So we may spread it.
