摘要
目的探讨糖尿病和胰腺癌的相关性。方法选取胰腺癌患者175例作为观察组,同期体检者192例作为对照组。抽取两组人群空腹和餐后血液进行血糖、胰岛素和C-肽检查,统计两组人群糖尿病发生率以及糖尿病病程;对观察组患者病理情况进行检测。结果观察组人群糖尿病发生率明显高于对照组,且观察组糖尿病患者病程<2年和≥10年者均明显多于对照组(P<0.05)。两组患者血糖水平差别不大(P>0.05),但观察组患者各个时间胰岛素水平明显高于对照组,C-肽水平明显低于对照组。上述比较两组差异,有统计学意义(P<0.05)。两组患者年龄差别不大,肿瘤部位差别不大,无统计学意义(P>0.05)。合并糖尿病的胰腺癌患者以乳头状癌或者高分化腺癌为主,而非糖尿病胰腺癌患者则以低分化腺癌为主,两组比较差异,有统计学意义(P<0.05)。结论糖尿病与胰腺癌之间存在着一定的关系,对新发糖尿病患者或者是病程较长的糖尿病患者进行胰腺功能筛查具有一定的理论基础,但随着糖尿病发病率的升高,其可行性有待进一步探讨。
Objective To investigate the correlation between diabetes and pancreatic cancer. Methods 175 patients with pancreatic cancer patients as the observation group,physical examination over the same period 192 cases as a control group. Check fasting and postprandial blood glucose,insulin and C-peptide extract two groups,the statistical the two populations incidence of diabetes and duration of diabetes;detection of patients with pathological conditions of the observation group. Results The population incidence of diabetes was significantly higher,and to observe the group of patients with diabetes duration of 2 years and ≥ 10 years caught significantly more than the control group(P 0.05). Two groups of patients with blood glucose levels are not very different(P 0.05),but the observation group were significantly higher insulin levels each time,C-peptide levels were significantly lower than the control group. The comparison between the two groups was statistically significant(P 0.05). The difference between the two groups of patients age,tumor location little difference was not statistically significant(P 0.05). Diabetic pancreatic cancer patients with papillary carcinoma or adenocarcinoma, non-diabetic patients with pancreatic cancer by poorly differentiated adenocarcinoma,the difference was statistically significant(P 0.05). Conclusion Between diabetes and pancreatic cancer there is a certain relationship,pancreatic function screening diabetic patients with new onset diabetes or longer course has some theoretical foundation,but with increasing incidence of diabetes,its possible sex to be further explored.
