鼻内镜下经口腔小儿腺样体切除术临床体会

Clinical experience of children's oral endoscopic adenoidectomy

目的:探讨70°鼻窦内镜下为儿童经口腔、鼻腔联合径路行腺样体切除术的优点。方法:30例儿童硬质70°鼻窦内镜下行经口腔入路腺样体切除术,分析其与传统匣式腺样体切除器切除法、腺样体刮匙刮除法及鼻内镜下单独经鼻腔入路腺样体切除术比较的优势。结果:30例患儿手术在内镜直视下进行,出血量约为10～20 mL,术中无损伤咽鼓管咽口及圆枕现象。术后当天最迟第2天睡眠中打鼾、憋气症状减轻甚至消失,疗效非常满意。结论:儿童经口腔鼻内镜下腺样体切除术具有直视明确,操作简单、方便,创伤小,疗效高之优点。

Objective ： To summary of the advantages of 70°oral endoscopic adenoidectomy for children adenoidectomy though oral cavity and nasal cavity. Methods：Oral adenoidectomy though oral cavity by 70~ rigid nasal endoscope for 30 chil- dren, and compared it with other three methods ： traditional removal device excision method, curettage curette method and nasal approach of adenoidectomy. Results：Thirty cases of surgeries were accomplished successfully with less bleeding 10 ~ 20 mL, and without injuring of pharyngeal opening of auditory tube or tubal torus. Than day and seconod day after surgery symptoms like snoring and apnea alleviated or disappeared. So the healing efficacy was satisfying. Conclusion：Children＇s endoscopic ade- noidectomy by oral approach is a intuitive, simiple and convenient method and with less trauma and higher efficacy.
