Discussion on the model of pharmaceutical care for patients with infective endocarditis

目的探讨心血管临床药师如何对住院患者开展临床药学监护。方法对1例感染性心内膜炎患者的药物治疗过程进行全程药学监护，建立药学监护模式。结果通过对患者的全程药学监护，不但解决了药物治疗过程中存在的问题，同时避免了药物治疗中的潜在风险。结论建立完善系统的药学监护模式，既提高了患者药物治疗的依从性及治疗效果，又减少了药物的相互作用及不良反应。

Objective To discuss how cardiovascular clinical pharmacists should carry out the pharmaceutical care for hospitalized patients. Methods The whole drug treatment process for one patient with infective endocarditis was monitored. The clinical pharmacists summarized experiences and established the model of pharmaceutical care. Results The clinical pharmacists not only solved the existing problems in the process of drug treatment, while at the same time a- voided the potential risks of the treatment through the entire pharmaceutical care to the patient and follow - up treatment. Conclusion The establishment of a systematic pharmaceutical care model improves patients compliance with therapy and the therapeutic effect, also reduces drug interactions and adverse drug reactions.
