改良悬雍垂腭咽成形术联合鼻部同期手术治疗重度阻塞性睡眠呼吸暂停低通气综合征分析

Modified uvulopalatopharyngoplasty combined with one stage nasal surgery for patients with severe obstructive sleep apnea hypopnea syndrome

目的探讨应用改良悬雍垂腭咽成形术(H-UPPP)联合鼻部同期手术治疗重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的可行性、优越性和注意事项。方法对28例重度OSAHS患者行H-UPPP,联合鼻中隔矫正、下鼻甲低温等离子消融术,全部患者均分别进行了术前和术后6个月、1年的多道睡眠图(PSG)监测。结果术后6个月、1年总有效率分别为100%、89.29%,无严重并发症发生。结论对腭咽平面及鼻腔平面阻塞所致的重度OSAHS患者,H-UPPP联合鼻部同期手术治疗是安全、有效的。

OBJECTIVE To explore the feasibility,advantages and precautions of modified uvulopalatopharyngoplasty combined with one stage nasal surgery in patients with severe obstructive sleep apnea hypopnea syndrome（OSAHS）.METHODS Twenty eight patients with severe OSAHS underwent modified uvulopalatopharyngoplasty,combined with the correction of deviated nasal septum and inferior turbinate hypothermy plasma ablation.All the patients underwent polysomnography before and 6,12 months after operation.RESULTS The total efficacy rate at 6 months and 1 year after operation was 100% and 89.29%.No serious complications occurred.CONCLUSION For severe OSAHS patients with obstructive level at velopharyngeal plane and nasal plane,modified uvulopalatopharyngoplasty combined with one stage nasal surgery is safe and effective.
