摘要
目的探讨金属植入物对射频消融的热影响。方法选取7个离体猪肝,分别建立银夹及125I粒子金属植入物模型。在距离金属植入物1.0cm处进行射频消融,以射频针为对称轴,将金属植入物一侧作为实验组,对侧作为对照组;术中采用测温针多点测温,比较两组间的时间-距离-温度关系,并对两组消融范围进行大体标本测量和光镜组织学分析。结果消融时间分别为8、12、15min时,实验组与对照组的多点平均温度分别为(48.24±18.07)℃和(48.58±17.69)℃、(51.47±19.57)℃和(52.24±19.73)℃、(54.64±19.75)℃和(54.94±19.24)℃,差异均无统计学意义(P均>0.05);12min与15min时,实验组消融范围分别为横径(1.55±0.12)cm、纵径(3.48±0.11)cm及横径(1.89±0.20)cm、纵径(3.72±0.16)cm,对照组分别为横径(1.56±0.12)cm、纵径(3.48±0.11)cm及横径(1.89±0.21)cm、纵径(3.72±0.16)cm,差异均无统计学意义(P均>0.05);两组消融灶光镜下表现相似。结论射频消融离体猪肝时,金属植入物不会造成明显热影响。
Objective To observe the thermal effect of metal implants on radiofrequency ablation(RFA).Methods Metal implants models were established with 7 in vitro porcine livers using silver clips or 125I seeds.RFA were conducted 1.0 cm away from them centering the RFA electrode axis,with one side containing metal implants model as experimental group and the other side as control group.The thermometric needles were used to measure multi-point temperatures in order to compare the time-distance-temperature difference between the two groups.The gross scopes of the ablation of the two groups were measured,and the tissues were analyzed for microscopic histology.Results At the ablation time of 8,12 and 15 min,the multi-point average temperatures of the experimental group and the control group were(48.24±18.07)℃,(51.47±19.57)℃,(54.64±19.75)℃ and(48.58±17.69)℃,(52.24±19.73)℃,(54.94±19.24)℃,respectively(all P0.05),while the ablation horizontal diameter of experimental group was(1.55±0.12) cm,(1.89±0.20)cm,longitudinal diameter was(3.48±0.11)cm,(3.72±0.16)cm and the ablation horizontal diameter of control group was(1.56±0.12)cm,(1.89±0.21)cm,longitudinal diameter was(3.48±0.11)cm,(3.72±0.16)cm,respectively(all P0.05).The two groups had the same manifestations under microscopy.Conclusion Metal implants do not cause significant thermal effect on RFA for in vitro porcine liver.
