The posterior ankle fracture treated with anterior medial incision

摘要 目的探讨前内侧入路治疗后踝骨折的优势及临床疗效。方法对58例涉及后踝的踝关节骨折患者采用前内侧切口治疗,予螺钉和(或)克氏针固定。术后石膏固定3~4周。结果 50例患者得到随访,时间4~38个月,骨折全部愈合。疗效按美国足踝AOFAS评分标准:优18例,良26例,中4例,差2例,优良率为88%。结论前内侧切口可以完整地观察胫距关节面,直视下对后踝进行复位固定,临床疗效较满意。 Objective To evaluate the clinical advantage and efficacy of anterior and medial approach for the treat- ment of posterior malleolar fractures. Methods 58 cases of ankle fracture were involved, using anteromedial incision posterior malleolar fractures, all were treated with screws or/and Kirschner wire fixation, with postoperative plaster immobilization for 3 ~ 4 weeks. Results A total of 50 patients were followed up for 4 ~ 38 months. All fractures healed. Assessed in accordance with the United States and AOFAS score standard, the results were excellent in 18 cases, good in 26, fair in 4, and poor in 1, the excellent and good rate was 88%. Conclusions Anteromedial incision can facilitate observation tibiotalar articular surface. Under direct vision of posterior malleolus for reduction and fixation, it is a more satisfied with clinical efficacy.
