MR的T2*WI序列在颅内血管性病变中的应用价值 被引量:1

摘要 目的探讨梯度回波T2*加权(GRE-T2*WI)序列在颅内血管性病变诊断中的临床价值。方法对本院收治并经临床确诊为颅内血管性病变的102例患者进行MRI扫描,并在常规MR序列(T2WI、T1WI、FLAIR)的基础上,行GRE-T2*WI序列扫描,比较诊断结果。结果经CTA诊断,102例患者有133处病灶,MR的GRE-T2*WI序列扫描均有显示,病灶检出率为100.0%,MR常规序列共诊出71例95个病灶(占71.4%),GRE-T2*WI序列诊断结果显著优于常规序列(P<0.05)。31例(共38个病灶)MR常规序列未检出病灶为:蛛网膜下腔出血慢性期11例(15个病灶),高血压性脑微出血12例(14个病灶)以及梗死继发性出血8例(9个病灶)。结论 GRE-T2*WI对各种出血性血管病变的诊断具有较高的敏感性,其颅内血管性病变总检出率优于MR常规序列。 Objective To explore the clinical value of gradient echo T2*weighted imaging (GRE-T2*WI) sequence in the diagnosis of intracranial vascular lesions. Methods 102 cases of patients whom were diagnosed as intracranial vas- cular lesions were scanned with MRI,and all the patients were scanned in the GRE-T2*WI sequence on the basis of the conventional MR sequences(T1WI,T2WI and FLAIR).The diagnostic result of the two sequences were compared af- ter scanning. Results There were 133 lesions among the 102 patients were diagnosed by CTA,and all the lesions were showed by the GRE-T2*WI sequence of MR,the lesion detection rate was 100.0%;while there were only 95 lesions a- mong the 79 patients were diagnosed by conventional sequence of MR,the lesion detection rate was 71.4%.The lesion detection rate of GRE-T2*WI sequence by MR was significantly higher than that of MR conventional sequence (P〈 0.05).The 31 cases (a total of 38 lesions) missed by MR conventional sequence were 11 cases (15 lesions) of chronic subarachnoid hemorrhage,12 cases(14 lesions) of hypertensive cerebral micro hemorrhage,and 8 cases(9 lesions) of in- farction secondary bleeding. Conclusion GRE-T2*WI has high diagnostic sensitivity to all sorts of hemorrhagic vascu- lar lesions,which diagnostic rate of intracranial vascular lesions is higher than that of the MR conventional sequence.
