高血压病患者血清PCⅢ、LN、HA的变化──苯那普利干预实验

摘要 目的 通过观察高血压病患者血清Ⅲ型前胶原(PCⅢ)、层粘连蛋白(LN)、透明质酸(HA)含量的变化,以及苯那普利对其影响,探讨上述参数作为心肌肥厚指标的临床价值。评价苯那普利对高血压性心肌纤维化的影响。方法测定64例高血压病患者在用苯那普利前后血清中PCⅢ、LN、HA水平的变化,并将左室心肌重指数(LVMI)与上述各参数作相关分析。结果 高血压伴左室肥厚组较高血压不伴左室肥厚组及正常对照组的血清PCⅢ、LN、HA含量显著增高,用苯那普利后伴与不伴左室肥厚的高血压患者上述指标均明显下降;血清PCⅢ、LN、HA均与LVMI呈显著正相关。结论 血清PCⅢ、LA、HA均可作为高血压左室肥厚患者组织纤维化的血清学指标,苯那普利可防治高血压患者心肌纤维化的发生。 Objective To investigate the changes of serum precollagen type Ⅲ(PCⅢ),Iaminin(LN)and hyaluronic acid(HA) in essential hypertensive patients,and influence of benazepril on these parameters. Methods serum PC Ⅲ、LN、HA were measured by radioimmunoassay in 64 essential hypertensive patients before benazepril treatment and after benazepril treatment for four weeks.The correlations between left ventricular mass index (LVM I)and the parameters above mentioned were analyzed. Results Serum PCⅢ,LN and HA were significantly increased in hypertensive patients with LVH as compared with those without LVH and healthy subjects. Serum PC Ⅲ, LN and HA were positively correlated with LVMI in the group of hypertensive patients. Serum PCⅢ、 LN、 HA were significantly decreased in hypertensive patients with or without LVH after benazepril treatment. Conclusion These findings suggest that measurements of PCⅢ, LN and HA may provide indirect diagnostic information on myocardial fibrosis associated with arterial hypertension, and benazepil may prevent hypertensive patients from myocardial fibrosis.
