摘要 目的通过检测TLR4及α-SMA在非酒精性脂肪性肝炎(NASH)患者肝组织中的表达情况探讨TLR4在发病机制中的意义。方法采用免疫组织化学ElivisionTMplus两步法的方法,检测60例不同病变程度NASH穿刺肝组织及10例正常肝组织中的TLR4表达,分析其与NASH病变以及与活化的肝星状细胞(HSC)的关系。结果正常肝组织中TLR4的表达主要位于个别库否细胞及星状细胞的胞膜及胞浆,在NASH组中,它们的表达均明显高于正常对照组(P<0.05)。连续切片免疫组化观察,α-SMA表达的星状细胞一部分表达TLR4,且TLR4与α-SMA的表达呈正相关关系。TLR4在NASH组中的表达与炎症活动程度分级和纤维化程度呈正相关关系。结论 TLR4在NASH疾病的发生发展中起了重要的作用,其在NASH中的作用很有可能是通过肝星状细胞的活化来完成的。 Objective To investigate the expressions of TLR4 and a-SMA in human non-alcoholic steatohepatitis (NASH) ,and to explore their potential roles in the pathogenesis of NASH. Methods Liver tissue obtained from 60 pa- tients pathologically diagnosed as nonalcoholic steatohepatitis and 10 healthy adult liver tissues were examined in the present study. The respective expression of TLR4 was determined at the protein expression levels by using immunohis- tochemical of ElivisionTM plus two-step method,Analysis the relationship between it and activation of hepatic stellate cell. Results In healthy adult liver tissue the expression of TLR4 appeared mainly in the cell membrane and cytoplasm of several hepatic stellate cell and kuffer cell. In NASH liver tissue,the expression of TLR4 is obviously higher than that of normal control group. (P^0.05). The expression was parallel to different degrees of inflammation in NASH , significant correlation was found between degree of inflammation andfibrosis and extent of expression (P〈0.05). Seri- al section observation:some of expressing a-SMA stellate cell is expressing TLR4,and correlation in the expression of a-SMA and TLR4 remains Positive correlation. Conclusion In the happening of the NASH disease development,TLR4 plays an important role . perhaps it is through the hepatic stellate cell activation to finish.
