生物反馈治疗出口梗阻型便秘

Clinical study of biofeedback therapy in patients with outlet obstructive constipation

目的研究生物反馈治疗对出口梗阻型便秘患者的临床症状、精神状态及生活质量的影响。方法选取功能性便秘患者42例,经肛门直肠测压后确认排便时存在矛盾运动,对每名患者进行5～10次生物反馈治疗后,应用便秘症状评估表、焦虑自评量表(SAS)、抑郁自评量表(SDS)及简明健康调查量表(SF-36)评估患者治疗前后的临床症状、精神状态及生活质量的变化。结果生物反馈治疗后,患者临床症状明显好转,症状积分量表中排便间隔天数、排便困难、排便阻塞感及总积分均有显著降低,总有效率约89%。患者焦虑、抑郁积分较治疗前显著降低(P<0.05),SF-36得分在生理职能、社会功能、精力、情感职能及精神健康5个维度积分明显增加(P<0.05)。结论生物反馈治疗能够改善患者便秘症状,缓解焦虑和抑郁的情绪,提高生活质量。

Objective To investigate the effects of biofeedback therapy on the clinical symptoms, mental state and quality of life in patients with outlet obstructive constipation. Methods 42 patients with functional constipation from the outpatients in Anhui Provincial Hos- pital were included in the study, and every patient was offered biofeedback therapy for five to ten times, and then the clinical symptoms were compared by symptoms scores . The mental state and quality of life were also evaluated by using self- rating anxiety scale （SAS） ,self- rat- ing depression scale （SDS）, and SF- 36 scale. Results After biofeedback therapy, clinical symptom of patients with FC was greatly im- proved ： there was a very significant decrease in total and subscales scores of bowel symptom including spontaneous frequency of bowel move- ments, straining effort , sensation of anorectal obstruction, and the total effective rate was 89%. The integration of SAS and SDS were obvious- ly decreased after treatment （P 〈 0. 05 ）, the SF - 36 scores were improved in five dimensions including role - physical, social functioning, vitality, role - emotional and mental health （P 〈 0.05）. Conclusion Biofeedback therapy has a greater value of clinical application for im- proving the clinical symptoms, alleviating the moods of anxiety and depression, and improving the quality of life in patients with FC.
