双螺旋CT在新生儿缺血缺氧性脑病中的应用价值

The application value of dual spiral CT in neonatal hypoxic-ischemic encephalopathy

目的探讨双螺旋CT在新生儿缺血缺氧性脑病中的应用价值。方法收集铜陵市第四人民医院2006年10月至2012年10月间64例确诊为缺血缺氧性脑病的患者,分析其CT图像,判断病变的部位、范围、有无合并颅内出血,并进行CT分度。采用SPSS 13.0统计软件包对CT分度与临床分度的一致性进行分析,P<0.05为差异有统计学意义。对以上患者进行随访复查,通过CT观察缺血缺氧性脑病的后遗表现。结果 64例中16例CT分度为轻度,33例中度,15例重度。CT分度与临床分度总的符合率为73.4%,经检验P=0.000,CT分度与临床分度有较高一致性;64例中28例合并颅内出血,蛛网膜下腔出血是最常见的出血类型;分度越重,出血发生率越高;27例进行了随访复查,其中11例正常,8例外周性脑积水,局限性脑萎缩4例,弥漫性脑萎缩3例,脑软化1例。结论双螺旋CT在缺血缺氧性脑病的诊断、分度、有无颅内出血中具有重要的价值,是缺血缺氧性脑病跟踪随访的有效手段。

Objective To explore the application value of dual spiral CT in neonatal hypoxic - ischemic eneephalopathy. Methods From October 2006 to October 2012, collected 64 eases of hypoxie isehemie eneephalopathy that was confirmed of in our hospital, analyzed the CT images to determine the site of lesion, range, whether intraeranial hemorrhage and made CT grading. Results 16 patients mild ,33 moderate, 15 severe in 64 cases of patients. The overall coincidence rate of CT grading and clinical grading was 73.4%, statistic result was P ：- 0.000 and show higher consistency of CT grading and clinical grading ; 28 of 64 patients with intracranial hemorrhage, sub- arachnoid hemorrhage is the most common type of bleeding; Higher the grading, the higher the incidence of bleeding. 27 cases of patients Conducted a follow -up review, including 11 normal, 8 peripheral hydrocephalus,4 partial atrophy of brain tissue, 3 diffuse atrophy of brain tissue, 1 softening of the brain. Conclusion The dual spiral CT has important value in the diagnosis of hypoxic - ischemic eucephalopathy, the grading, whether intracranial hemorrhage, The CT is a kind of effective method for followed - up.
