腹膜透析患者用药管理调查

A Survey on Medication Management in Peritoneal Dialysis Patients

目的调查腹膜透析患者用药管理的基本情况,为开展针对性的药学服务提供参考。方法对本院定期随诊的腹膜透析患者,采取问卷及访谈的方式调查患者的用药依从性、药物知识的掌握情况、药物治疗效果、药学服务的期望。结果共调查者58例,平均用药(6.7±2.6)种,日均口服药费用达(36.69±25.93)元。87.9%的患者用药依从性较差,主要表现为忘记服药或未按时、按量服药。药物治疗效果欠佳,血红蛋白达标率仅为15.52%,iPTH达标率仅为21.15%。86.20%患者通过医院获取药物知识,84.48%患者仅关注药物治疗的效果。结论腹膜透析患者用药依从性差,治疗效果不理想,普遍缺乏药物知识。临床药师应加强与患者沟通,开展有针对性的药物知识宣教并定期随访,以提高患者用药依从性及药物治疗效果。

Objective To investigate the status of medication management in peritoneal dialysis patients, so that the clinical pharmacists could provide targeted pharmaceutical services for patients. Methods Peritoneal dialysis patients at return visit in the hospital were investigated with questionnaires and interviewed to understand their medication compliance, drug knowledge, drug therapy and requirements of pharmaceutical care. Results A total of 58 peritoneal dialysis patients were investigated. Mean numbers of concomitant oral drugs were 6.7 ± 2.6. Average daily oral drug treatment costs were （36.69 ±25.93） yuan. 87.9% of the patients with bad compliance during the 58 patients. Forgetting to take medicine or taking on wrong time were very common. The hemoglobin levels of only 15.52% of the patients were in the target range. 86.20% of patients with uncompleted medicine knowledge got drug information only by the doctorg advice. 84.48% of patients only paid attention to the effect of drug treatment. Patients mainly depended on the doctors. Only 1.72% of patients preferred education provided by pharmacists. Conclusion Peritoneal dialysis patients are generally lack of drug knowledge and awareness of the clinical pharmacists. They were always with poor medication compliance and treatment outcome. Clinical pharmacists should strengthen the communication with the patients, and take targeted measures on undergoing drug education and conduct regular follow-up in order to improve drug compliance of patients and the treatment outcome.
