摘要
本文从医生道德风险的成因入手,并构建动态博弈模型,运用信息经济学有关理论分析其机理和表现形式,并从第三方角度,提出相应的规避策略,首先就医生道德风险的羊群效应的表现形式进行描述,然后基于动态博弈模型研究了单个医生道德风险的产生,进而推广到医生群体,研究了羊群效应的产生机理。在此基础上提出了规避不合规的羊群效应、诱导合规羊群效应的主要途径:1)降低医生的违规收益;2)加大对医生的违规惩罚;3)提高违规行为的发现概率;4)加强医生医德医风建设,形成良好的医院文化。
From the causes for doctor moral hazard, this paper set the dynamic game model, used the theory of information economics to analyze the mechanisms and manifestations, and proposed the corresponding avoidance strategies from the third party perspective. Firstly, the representation of herding effects of moral hazard among doctors is described, then the causes for the moral hazard of single doctor is studied based on the dynamic game theory model, and further extended to the group of doctors, based on which the cause mechanism of herding effects is studied. According to the discussion above, the main approaches are proposed to avoid the herding effects that violating the rules and to induce the herding effects that obey the rules: 1) reducing the benefits of doctors when violate the rules; 2) enhancing the punishment of doctors when violate the rules; 3) increasing the probability of discovering the behaviors that violate the rules; 4) strengthening the morals construction of doctors to forth good culture of hospitals.
