白内障超声乳化术后早期浅前房原因及处理

Causes and management of early shallow anterior chamber after phacoemulsification

探讨防止超声乳化白内障摘除术后浅前房所引起的眼部损害的方法。方法 :对 18例超声乳化白内障摘除术后浅前房的病例进行观察 ,分析其原因及处理方法。其中 17例为老年性白内障 ,1例为先天性白内障 ,术中通过颞侧透明角膜隧道切口行超声乳化白内障摘除及折叠式人工晶体植入术 ,观察术后前房的深浅、视力及眼压变化。结果 :18例浅前房中 ,9例为切口漏水 ,8例为瞳孔阻滞 ,1例为迟发性脉络膜上腔出血。切口漏水组须再次手术缝合切口 ;瞳孔阻滞组用药物散瞳及抗炎治疗 ,仅 1例需手术治疗 ;迟发性脉络膜上腔出血者则对症处理。 15例 (83.3% )病人术后 2周视力均恢复至 0 .5以上。结论 :及时的对症处理 ,可以防止超声乳化白内障摘除术后浅前房对眼部的损害 ,使病人得到较好的视力恢复。

To search for the methods for prevention of ocular damages resulting from shallow anterior chamber after Phacoemulsification.Methods:Eighteen cases of shallow anterior chamber after phacoemulsification were observed.Seventeen cases were senile cataract patients and one was congenital cataract child.Surgical techniques were temporal clear corneal tunnel incision,phacoemulsification and foldable intraocular lens implantation.The causes and management methods of shallow anterior chamber were analyzed.Results:Shallow anterior chambers were caused by leakage of the incision in 9 cases,by pupillary block in 8 cases,by late suprachoroidal bleeding in 1 case.Suturing of incision was carried out in leakage patients.Medicinal dilation of the pupil and antiinflammatory treatment as well as surgical intervention(only in 1 case)were used for pupillary blocking patients.Conservatory treatment was used for the suprachoidal bleeding patient.Two weeks after the surgery,15 cases(83 3%)obtained visual acuity of 0 5 or better.Conclusion:Prompt management can prevent postoperative shallow anterior chamber from further damaging the eye and assure good visual rehabilitation.
