123例穿通性角膜移植临床观察

Clinical observation of 123 cases penetrating keraoplasty

目的 观察穿通性角膜移植的治疗效果 ,探讨影响成功率的因素。方法 采用新鲜角膜材料行穿通性角膜移植 146例 ( 158眼 ) ,术后 12 3例 ( 12 8眼 )随访观察 3个月至 5年 (平均 1.5年 )。结果 植片透明愈合率为 71.1% ,57眼 ( 4 4.5% )裸眼视力为 0 .0 5～ 0 .3,4 2眼 ( 32 .8% )视力达 0 .3以上。结论 穿通性角膜移植是治疗多种角膜疾病、恢复视力的非常有效的方法 ,手术成功率取决于术眼条件 ,供体质量 ,手术技术和并发症处理。

Objective To observe the therapeutic effect of penetrating keratoplasty (PKP) and explore the factors affecting the success rate of the operation. Methods PKP was performed on 146 cases (158 eyes) with fresh donor corneas. After the operation, 123 cases (128 eyes) had been followed up for 3 months to 5 years (mean duration 1.5 years). Results The rate of graft clarity was 71.1% (91/128), 57 eyes (44.5%) had the uncorrected visual acuity of 0.05 to 0.3 and 42 eyes (32.8%) even better than 0.3. Conclusion PKP is an outstanding approach to treat various corneal disorders and recover the visual acuity. The success rate of PKP depends on the condition of operated eyes, the quality of donor corneas, surgical skills and complication management.
