摘要
目的:为积极配合2011年卫生部、北京市卫生局关于抗菌药物临床应用专项整治工作,调查了解我院门诊抗菌药物使用情况,并规范其合理使用,为临床用药提供参考。方法:以随机抽样法抽取2013年4 d门诊西药处方,依据《抗菌药物临床应用指导原则》、《医院处方点评管理规范(试行)》及药品说明书,对应用抗菌药物的处方进行分类、统计与分析。结果:共抽查处方4 841张,其中应用抗菌药物处方1 081张,存在抗菌药物不合理用药处方316张,不合理用药主要表现在无指征用药、超说明书用药、用法与用量不合理、溶剂选用不当、重复用药、药理性拮抗及不良反应相加等方面。结论:临床药师应对临床不合理用药进行干预,以提高临床处方的合格率,加强抗菌药物的管理,规范抗菌药物的合理使用。
OBJECTIVE ：To carry out the special rectification activities of antibacterials issued by Beijing Ministry of Health and Bejing Municipal Health Bureau, investigate the outpatient antibiotic use in our hospital and standardize the antibiotic use so as to provide reference for clinical medication. METHODS： The outpatient prescriptions of western medicine selected randomly in 4 days of 2013 were classified and analyzed statistically with regard to antibiotic use in accordance with ＇ Guiding principles for clinical application of antibacterials＇ , ＇ Management Practices of Hospital Prescription Comment （Trial） ＇ and drug package inserts. RESULTS ： A total of 4 841 prescriptions were reviewed, of which, 1 081 were antibiotic prescriptions, 316 prescriptions involved non-rational use of antibiotics, and the non- rational use manifested as non-indication medication, off-label use, irrationality in administration or choice of solvents, repeated medication, pharmacological antagonism and superimposed side effects etc. CONCLUSION： It is important for clinical pharmacists to adopt intervention on clinical non-rational use of drugs in an attempt to improve the acceptability rate of clinical prescriptions, strengthen management on antibiotics and standardize the rational use of antibiotics.
