运动疗法治疗青年人颈型颈椎病的效果观察

Therapeutic effect of kinesitherapy on the young patients suffered from neck type cervical vertebra disease

目的观察运动疗法对青年人颈型颈椎病的疗效。方法所有394例颈型颈椎病青年患者均行颈部单一方向运动疗法,观察每动持续时间、极限位保持时间、每次运动时间、疗程、去除病因与否、性别、年龄,病程等因素对该运动疗法的疗效影响,并采用多元回归方程分析。然后在相同的条件下选取颈部复合方向运动疗法20例及颈部单一方向运动疗法20例进行疗效比较,疗效比较采用Ridit分析。结果颈部单一运动疗法每动持续时间在2 s以上时,所有患者均无任何不适;极限位保持时间、每次运动时间、去除病因与否、疗程长短,方程系数均>0,提示这些因素与运动疗法疗效呈正相关;病程长短方程系数也<0,与运动疗法疗效呈负相关。而每动持续时间及性别、年龄与运动疗法疗效无统计学上的相关性。在条件相同的情况下,单一方向运动疗法与复合方向运动疗法相比疗效无统计学差异(P>0.05)。结论运动疗法运动到极限位保持时间越长,每次运动时间越长,疗程越长,病程越短并去除病因,则对青年人颈型颈椎病疗效越好。

Objective To study the effect of kinesitherapy on the young patients suffered from neck type cervical vertebra disease. Methods The 394 patients with this disease received simple kinesitherapy, the influences of continuous time for each action, keeping time for cervical limit posture, exercise time, period of treatment, whether or not removed pathogenesis, sex, age, and disease course on the curative effects of the kinesitherapy were observed. The data was analyzed by multivariate regression analysis. Then the curative effects of multiple kinesitherapy（n=20） were compared with that of simple kinesitherapy（n=20） under the same condition. The comparison of curative effects was analyzed by Ridit analysis. Results When each action lasted more than 2 seconds, all the patients received simple kinesitherapy had no discomfort. For keeping time for cervical limit posture, exercise time, whether or not removed pathogenesis, treatment course, the equation coefficients all were more than 0, which suggested that those variables were related to curative effects of kinesitherapy positively and statistically for disease course, the equation coefficient was less than 0 which was related to curative effects of kinesitherapy negatively and statistically. There was no statistical differences in curative effects between simple kinesitherapy and multiple kinesitherapy at same condition. Conclusion The more continuous time for each action, keeping time for cervical limit posture, exercise time, treatment course were spent, and pathogenesis was removed, the less disease course was, the better curative effect of kinesitherapy on young patients suffered from neck type cervical vertebra disease had.
