补气活血养股汤结合介入疗法治疗无菌性股骨头坏死症40例临床观察

A Clinical Observation on Qi-Supplementing Blood-Quickening Femur-Nourishing Decoction Combined with Interventional Therapy for 40 Cases of Avascular Necrosis

目的观察补气活血养股汤结合介入疗法治疗早中期无菌性股骨头坏死症(ATBD)气虚血瘀兼肝肾亏虚证的临床疗效及安全性。方法 80例ATBD气虚血瘀兼肝肾亏虚证患者随机分为治疗组(给予介入疗法及中药补气活血养股汤)和对照组(仅给予介入疗法)各40例,观察两组患者中医及西医的临床疗效,治疗前后进行Harris评分及中医症状评分,并观察不良反应。结果治疗组西医疗效总有效率为82.50%,对照组为70.00%,治疗组优于对照组(P<0.05)。治疗组中医疗效总有效率85.00%,对照组为65.00%,治疗组优于对照组(P<0.05)。两组治疗后Harris评分比较,治疗组各项评分及总分均较对照组明显提高(P<0.05)。治疗后治疗组中医症状评分各项评分及总分均较对照组降低明显(P<0.05或P<0.01)。两组患者不良反应及手术并发症比较差异无统计学意义(P>0.05)。结论补气活血养股汤结合介入疗法治疗早中期ATBD气虚血瘀兼肝肾亏虚证疗效肯定,能明显改善临床症状,且不良反应少。

Objective To observe the efficacy and safety of Qi-Supplementing Blood-Quickening Femur-Nourishing Decoction（QSBQFND） combined with interventional therapy for early-middle stage avascular necrosis（AVN）.Methods Eighty early-middle stage AVN patients with qi deficiency blood stasis and liver-kidney deficiency pattern were randomized into the treatment group and control group,with 40 in each.The treatment group was given QSBQFND and interventional therapy.The control group was given interventional therapy.The efficacy of TCM and western medicine,Harris Hip Score（HHS）,TCM symptom scores and adverse reactions were observed.Results The efficacy of western medicine in the treatment group and control group was 82.50% and 70.00% respectively,with a significant difference between groups（P〈0.05）.The efficacy of TCM in the treatment group and control group was 85.00% and 65.00% respectively,with a significant difference between groups（P〈0.05）.Comparing with the control group,the scores of HHS in the treatment group were significantly increased（P〈0.05）.Comparing with the control group,the TCM symptom scores in the treatment group were significantly decreased（P〈0.05 or P〈0.01）.There was no significant difference between groups in the adverse reactions and surgical complications（P〈0.05）.Conclusion QSBQFND combined with interventional therapy is effective for early-middle stage AVN and it can obviously relieve the symptoms.
