我院血管紧张素受体拮抗剂用药情况分析

Angiotensin Receptor Blockers in Our Drug Utilization Analysis

目的分析我院内科门诊血管紧张素受体拮抗剂(ARB)用药情况,针对存在的问题提出干预、改善措施,以达到合理用药。方法抽取本院2011年10月1—31日门诊内科诊断为高血压的电子处方。再从中选出使用ARB治疗高血压的处方,分析ARB使用情况。结果经过医院信息系统(HIS)查询诊断含"高血压"的处方1195张,处方中含ARB的有440张,使用ARB治疗高血压的比例达36.82%。440张ARB处方中,合理处方189张,不合理处方251张,合格率仅为42.85%。不合理情况:用药频次不当119张(占47.41%),ARB联合β受体阻滞剂94例(占37.45%),联用该通道阻滞剂(CCB)使用不当63张(占25.10%),超门诊30d用量44张(占17.53%),ARB单次使用超最大剂量4张(占1.59%),合并症联合用药选用不当1张(占0.40%),合并使用抗生素用法不当1张(占0.40%)。结论 ARB临床使用率较高,但是正确使用率却比较低。

Objective Analyzing the situation of AItB using for high blood pressure of our internal medicine depart- ment, finding the problems, and offering a proposal for rational use of ARB. Methods Extracting prescriptions of high blood pressure from Oct. 1st - 31th, 2011 of our internal medicine department, then selecting the prescriptions of using ARB, and evaluating whether reasonable or not. Results After a hospital information system （HIS） query diagnosis contains ＂ hyperten- sion＂ prescription 1195 dmng, the prescription contains ARB have 440 pieces, using ARB treatment for high blood pressure ratio of 36. 82%. 440 ARB prescription, the reasonable prescription 440 zhang, unreasonable prescription 251 zhang, percent of pass is only 42. 85%. Unreasonable situation： improper drug use frequency of 119 （47.41%）, ARB combined beta -blockera in 94 cases （37. 45% ）, improper combination the channel blocker （CCB） using 63 accounts for （25. 10% ）, super outpatient dosage of 30 d 44 （ 17. 53% ）, ARB single use ultra dose four largest （ 1.59% ）, complications combination improper selection 1 （0. 40% ）, improper use of antibiotics usage 1 （0. 40% ）. Conclusion ARB clinical utilization rate is higher, but the cor- rect usage is low.
