摘要
目的观察B型超声评价胆囊功能在微创保胆取石术中应用的效果。方法 372例胆囊结石患者据B型超声声像图特点分为甲组222例和乙组150例。2组均在B型超声指导下采用腹腔镜和输尿管镜微创手术进行保胆取石治疗。观察2组临床疗效及术后1、3、6个月和1、2年时的随访复发情况。结果患者手术均成功,成功率为100%。无中途剖腹病例。甲组随访1、3、6个月及第1年均无复发病例,随访第2年复发2例(0.9%),均为多发结石。乙组随访1、3、6个月均无复发病例,随访第1年复发2例,均为多发结石,随访第2年复发6例,其中单发2例,多发4例。总计8例(5.3%),2组比较差异有统计学意义(P<0.05)。结论采用B型超声对胆囊结石患者的胆囊功能进行有效的评价,并指导腹腔镜联合输尿管镜进行微创手术,对患者成功进行保胆取石有较大的临床价值,值得推广应用。
Objective To observe the efficacy of B-mode ultrasound evaluated of gallbladder function in minimally in vasive gall-preserving lithotomy. Methods According to the characteristics of B type ultrasonography,372 cases patients with cholecystolithiasis were randomly divided into Group A (222cases) and Group B(150cases). All patients were performed lapa- roscope and ureteroscopic minimally invasive operation in order to cholecystolithotomy in the guidance of B type ultrasound. Af ter surgery,compared the clinical efficacy of 2 groups,and observed the fullow-up recurrence of 1,3,6 months and 1,2 years. Results All patients were cured, the success rate was 100%. No cases of caesarean section midway. No follow-up recurrence in 1,3,6 months and lth year of group A. 2 cases followed-up recurrence in 2th year of group. The 2 cases followed-up recur rence of group A were multiple stones. No followed-up recurrence in 1,3,6 months of group B,2 cases followed-up recurrence in lth year of group B and were multiple stones. 6cases followed-up recurrence in 2th year of group B. 2 cases were single stones. 4 cases were multiple stones,a total of 8 cases(5.3% ) ,there was no statistically significant difference compared to 2 groups. Conclusion Evaluate gallbladder function of patients with gallstones by B-made ultrasound,instruct laparoscopic and ureteroscopy for minimally invasive surgery, have a greater clinical value for patient with Mini-cholecystolithotomy, worthy of clinical application.
