腹腔镜子宫肌瘤剔除术对机体免疫功能的影响机制研究

目的研究腹腔镜子宫肌瘤剔除术对机体免疫功能的影响机制。方法将2011年1月～2013年1月在我院行腹腔镜子宫肌瘤剔除术的30例患者设立为观察组,另选择同期行开腹子宫肌瘤切除术的30例患者为对照组,观察两组患者的手术时间、术中出血量、术后排气时间、并发症情况及两组患者术前术后IgA、IgG、IgM的变化情况。结果与对照组比较,观察组患者的手术时间(92.12±21.75)min明显长于对照组,但术中出血少、术后排气快、并发症少,两组比较差异有统计学意义(P<0.05)。术后对照组患者的IgA、IgG、IgM较术前明显降低,差异有统计学意义(P<0.05),而观察组上述指标术前术后未见明显变化(P>0.05),且对照组术后3 d的IgA、IgG、IgM均低于观察组,差异有统计学意义(P<0.05)。结论与开腹子宫肌瘤切除术相比,腹腔镜子宫肌瘤切除术具有创伤小、恢复快、并发症少等优点,考虑可能与腹腔镜手术对机体免疫功能抑制少有关。

Objective To study the laparoscopic uterine fibroids myomectomy on the influencing mechanism of im- mune function. Methods From January 2011 to January 2013, 30 patients with laparoscopic uterine fibroids myomec- tomy in our hospital were established as the observation group, and the other 30 patients with concomitant abdominal hysterectomy myomectomy established as control group, the operative time, blood loss, postoperative exhaust time, complications and the IgA, IgG, IgM before and after the treatment were compared. Results Compared with the control group, the operation time of the observation group patients was （92.12±21.75） min, was significantly longer than the con- trol group, but less blood loss, faster postoperative exhaust, fewer complications, the differences between the two groups were significant （P 〈 0.05）. The postoperative IgA, IgG, IgM of control group were significantly lower than the preoper- ative, the difference was significant （P 〈 0.05）, while these indicators of observation group had no significant difference （P 〉 0.05）, and postoperative IgA, IgG, IgM were significantly lower than the observation group, the difference was sig- nificant （P 〈 0.05）. Conclusion Uterine fibroids and open resection compared laparoscopic myomectomy with less trauma, quicker recovery, fewer complications, etc., to consider laparoscopic surgery may be less suppression of im- mune function
